DaVita is looking for a Licensed Social Worker (LMSW) to support our patients and their families at our Sioux Falls (Sioux Falls, SD), and Moccasin Creek (Aberdeen, SD) Dialysis Clinics.

• Split coverage between Sioux Falls and Moccasin Creek Dialysis Clinics
• Monday - Friday schedule
• Full time status - 30 hours a week

• Life on dialysis can be a difficult transition for many patients - and you'll be there to support and advocate for them.
• Help patients understand their rights and responsibilities, and guide them in managing the physical, mental, emotional and financial demands of End Stage Renal Disease.
• Build meaningful and long-term relationships with patients and their families in an intimate outpatient setting.

Requirements

• Licensed or registered in the state of practice as required by state regulations - LMSW or LCSW
• Meets all state required regulations to practice in the dialysis setting.
• Master's degree in Social Work (MSW) required with a specialization in clinical practice
• Demonstrated knowledge of government and private insurance programs
• Basic computer skills in MS Word, Excel, PowerPoint and Outlook as well as functional proficiency with DaVita specific applications within 60 days
