Responsibilities:
- Visit patients in the home and assess their plan of care
- Provide care in your role as a Nurse Practitioner with a focus on quality and comfort
- Submit patient records and documentation in a timely manner
- Collaborate as part of the Interdisciplinary Care Team
Qualifications:
- Master’s Degree in Nursing
- Licensed Nurse Practitioner in the state of California
- Licensed and insured driver with reliable transportation
- Understanding and acceptance of hospice principles of care
- Strong knowledge of medicine, pharmacology, pain and symptom control, and patient admissions criteria
