SUMMARY OF POSITION:
Responsible for coordinating the individualized care continuum for patients and for achieving quality, cost-effective and patient-focused outcomes by serving as an advocate, educator, consultant, leader and community ambassador for a specific patient population in accordance with standards and guidelines. Provides consistent care coordination throughout the continuum of care and is able to assess the physical, psychological and social needs of the patient. Work is strongly focused on guiding the patient/family through the complex healthcare system, working to eliminate barriers that might otherwise adversely impact patient care/outcomes and helping to coordinate the efforts of the medical team to facilitate continuity of care and enhance patient satisfaction.
MINIMUM QUALIFICATION(S):
- Bachelor's Degree
- Minimum one (1) year of experience
