About the Role
Responsibilities
- Provide skilled physical therapy treatments under the direction of a licensed Physical Therapist and according to the established plan of care.
- Deliver therapeutic exercises, gait training, balance activities, transfer training, and mobility interventions to help residents achieve their functional goals.
- Monitor patient progress and communicate changes in condition to the supervising Physical Therapist.
- Accurately complete documentation and treatment notes using an electronic medical record (EMR) system.
- Educate residents and caregivers on home exercise programs, fall prevention, and safety strategies
- Collaborate with Physical Therapists and interdisciplinary team members to ensure coordinated, patient-centered care
- Maintain a full-caseload while providing compassionate, high-quality therapy services
Qualifications
- Current State Physical Therapy Assistant (PTA) License
- Current CPR/BLS Certification
- Valid driver’s license, reliable transportation, and auto insurance
- Previous experience in geriatrics, home health, assisted living, or skilled nursing is preferred but not required
