ACT Psychiatric Nurse (RN)
Location: Chicago, IL 60640 (On-site)
Job Type: Full-Time
Salary: $85,000 per year

We are seeking a compassionate Registered Nurse (RN) to join our Assertive Community Treatment (ACT) team. In this role, you'll provide community-based psychiatric nursing services to adults living with serious mental illness while collaborating with an interdisciplinary team dedicated to promoting recovery and independence.
Responsibilities
- Coordinate medical care and annual health evaluations for ACT clients.
- Administer injectable psychotropic medications under psychiatric direction.
- Monitor lab results, vital signs, medications, and medical appointments.
- Coordinate with physicians, pharmacies, hospitals, and community providers.
- Conduct home visits as needed and respond to medical emergencies.
- Participate in treatment planning, clinical meetings, and maintain timely Medicaid Rule 132/140 documentation.
- Provide education and support to clients, caregivers, and staff.

Qualifications
- Current Illinois Registered Nurse (RN) license.
- Minimum of 2 years' experience working with individuals with serious mental illness.
- Valid Illinois driver's license and ability to travel within the community.
- Ability to pass a background check and complete required annual training.
