Description
Provides leadership and supervision of the day-to-day clinical and administrative operations of the care management team. Supervises Care Management (CM) staff to include nursing, social work, patient care coordinators and other non-clinical staff to achieve the goals for the organization; collaborates directly with inpatient and ambulatory clinicians and care teams, as well SMG and PCN leaders to support cost-effective high-quality medical services.

Required qualifications:
Washington Registered Nurse License upon hire
5 years clinical nursing, including acute care hospital and/or ambulatory clinic experience.
Preferred qualifications:
Bachelor’s Degree
3 years Case Management experience including care management, care coordination, discharge planning, or population health.
2 years progressive supervisory experience.

The successful candidate's placement on the wage range for this position will be determined based upon relevant job experience and other applicable factors. These amounts are the base pay range; additional compensation may be available for this role, such as shift differentials, standby/on-call, overtime, premiums, extra shift incentives, or bonus opportunities.
