INTRODUCTION
Harbor is currently looking to add a Case Manager (CPST/TBS Provider) to our Toledo team. Experience with youth is preferred, and the position is required to work one late day per week.
Position is full-time, 40 hours per week. Pay starts at $16.89.
BASIC QUALIFICATIONS/REQUIREMENTS:
- Bachelor’s degree in Counseling, Social Work, Psychology, or a closely related behavioral/mental health field required, or, in lieu of a degree, a minimum of a high school diploma and 3 years of relevant experience.
- Must be proficient and accurate in computer use, including Microsoft Word.
- Must successfully complete required training, before services are rendered, in basic competencies including, at a minimum, an understanding of mental illness or substance use disorder treatment and recovery, psychiatric and substance use disorder symptoms and their impact on functioning and behavior; how to therapeutically engage a person with mental illness or a person in substance use disorder treatment and recovery; crisis response procedures; an understanding of the community behavioral health system, social service systems, criminal justice system and other healthcare systems; de-escalation techniques and understanding how your own behavior can impact the behavior of others.
- Must successfully demonstrate the basic competencies listed above, as determined by your supervisor’s review, within 90 days of hire.
- Must have a valid driver’s license, acceptable driving record, and show proof of personal auto insurance policy to be deemed insurable with Harbor’s vehicle insurance carrier.
- For specialized programs:
- Developmental Pediatrics: Completion of a 40-hour Registered Behavior Technician course required or to be obtained within 120 day introductory period.
- Substance Use Disorder Program: May require completion of a 40 hour State of Ohio Chemical Dependency Counselor Assistant certification.
- When applicable, non-licensed staff will be required to complete 20 hours of CEUs every 24 months.
- CPR/First Aid and NCI/CPI Certifications required within 90 days of employment.
ESSENTIAL JOB COMPETENCIES/PRIMARY DUTIES:
Documentation:
- Responsible for the quality and completeness of the clinical record.
- Develops and updates Individualized Service Plan (ISP) for each client.
- Responsible for accurate and timely documentation which may include but is not limited to activity/progress notes, ISPs, incident report forms, and any other forms necessary to document services.
- Completes clinical documentation in the format appropriate and acceptable to Harbor.
- In providing billable services, meets or exceeds unit expectations established by Harbor.
Provides Community Psychiatric Supportive Treatment service activities:
- Evaluation
- Assesses client needs and formulates a strength-based ISP in conjunction with the client, family and/or significant others.
- Evaluates and revises the ISP according to the client’s responses and needs.
- Crisis prevention/intervention
- Follows crisis prevention/intervention protocols.
- Ongoing interventions
- Provides support, education and consultation to clients, families, and/or significant others.
- Provides therapeutic interventions as noted in the ISP to maintain the client in the least-restrictive environment and to enhance client and family functioning.
- Monitors clients to identify and minimize effects of psychiatric symptoms.
- Provides education on self-management of symptoms to clients, families and/or significant others.
- Collaboration
- Assists the client and family in increasing social support skills and networks.
- Coordinates indicated evaluations and assessments by internal and/or external providers.
- Coordinates and monitors all services identified in the ISP.
- Provides assistance in gaining access to essential community resources.
Provides individual and group Therapeutic Behavioral services, including but not limited to:
- Treatment Planning.
- Identification of strategies or treatment options.
- Developing and providing solution focused interventions and emotional and behavioral management drawn from evidence-based psychotherapeutic treatments.
- Restoration of social skills.
- Restoration of daily functioning.
- Crisis prevention and amelioration.
- Group services.
