Facility: Ascension Saint Thomas West
Department: Advanced Heart Failure Clinic
Schedule: Full-time | Day Shift
ROLE AND RESPONSIBILITIES
How you'll make an impact in this role:
- Assess patients with chronic or acute heart failure through physical exams, symptom review, health history evaluation and monitor vital signs and weight trends.
- Educate patients and families on heart failure management, medication adherence, and lifestyle changes.
- Coordinate care plans with providers, including titrating medications per protocols.
- Manage incoming calls to assess symptoms, adjust care or refer to a physician.
- Act as a liaison between physicians and patients, facilitating follow-up appointments and diagnostic tests.
BASIC QUALIFICATIONS
What minimum qualifications you'll need:
Licensure / Certification / Registration:
- BLS Provider obtained within 1 Month (30 days) of hire date or job transfer date required. American Heart Association or American Red Cross accepted.
- One or more of the following required:
+ Licensed Registered Nurse credentialed from the Tennessee Board of Nursing OR current home state license for multi-state license recognition "Compact State" obtained prior to hire date or job transfer date. License eligible may be accepted.
Education:
- Diploma from an accredited school/college of nursing OR Required professional licensure at time of hire.
