INTRODUCTION
Complete Care is hiring an External Care Manager (ECM) to support our Chicago market across three skilled nursing facilities. This role is ideal for someone who thrives in a fast-paced, field-based environment and enjoys building strong relationships with hospital partners.
As an ECM, you will play a key role in driving admissions by coordinating patient transitions from hospitals to our facilities.
RESPONSIBILITIES for External Case Manager (ECM):
- Build and maintain relationships with hospital case managers, discharge planners, and social workers
- Serve as the main point of contact between hospitals and Complete Care facilities
- Coordinate patient referrals and ensure smooth transitions to post-acute care
- Conduct daily in-person visits to hospitals in the Chicago area
- Partner with admissions, clinical, and leadership teams to support census goals
- Track referral activity and identify opportunities for growth
QUALIFICATIONS for External Case Manager (ECM):
- Experience in healthcare admissions, marketing, case management, or hospital liaison role
- Skilled in relationship-building and communication
- Knowledge of hospital discharge planning and post-acute care preferred
- Self-motivated and able to work independently in the field
- Willingness to travel locally within the Chicago region
- Clinical background (RN/LPN) is a plus, but not required
