We are seeking a RN Field Case Manager to join Atlanta office.
This is primarily a field position with occasional in office time, as needed to ensure the highest quality of client care. Your Day-to-Day
- Manage and coordinate home care cases, ensuring clients receive exceptional service and support.
- Serve as the main liaison between clients, caregivers, and referral sources.
- Oversee daily office operations, assisting with scheduling, authorizations, and client communications.
- Conduct aide orientations, trainings, and supervisory visits to promote quality and consistency in care.
- Monitor compliance with all AccordCare policies, as well as state and federal regulations.
- Collaborate with leadership to continuously improve client satisfaction and caregiver performance.
What You’ll Bring
Current RN license in the State of Georgia (required)
1+ year of experience in home care or community health preferred
Excellent communication, leadership, and organizational skills
Positive, professional attitude and a strong commitment to quality care
Reliable transportation and valid driver’s license for occasional field visits
Familiarity with AlayaCare software a plus
