Job Summary

We are seeking an organized, dependable, and compassionate Registered Nurse Case Manager to support clients receiving in-home nursing services. This role focuses on completing intake and renewal documentation, overseeing clinical records, reviewing nursing visit notes, and helping ensure compliance with agency and program requirements.

The ideal candidate has strong documentation and time-management skills, works well independently, and understands Georgia's GAPP (Georgia Pediatric Program) or is willing to learn its requirements. This position requires accurate use of our electronic systems, reliable communication with agency leadership and clinical staff, and attendance at a monthly in-person meeting with the CEO.

Key Responsibilities

• Complete and maintain accurate client intake and renewal packets, including required clinical assessments, plans of care, supporting documentation, and signatures.
• Monitor renewal deadlines and submit complete documentation within required timeframes to prevent interruptions in client services.
• Review skilled nursing visit notes and supervisory visit notes for accuracy, completeness, clinical appropriateness, and compliance.
• Approve documentation that meets agency and program standards and return incomplete or inaccurate notes to the appropriate staff member for correction.
• Conduct required nursing assessments, supervisory visits, reassessments, and follow-up activities according to agency policies and applicable regulations.
• Develop, review, and update individualized plans of care in coordination with families, caregivers, physicians, field nurses, and other members of the care team.
• Maintain organized, accurate, and confidential client records while complying with HIPAA, professional nursing standards, and agency policies.
• Assist with documentation and clinical requirements related to GAPP, including intake, prior authorization, renewals, and continued service needs.
• Communicate with nurses, families, providers, and agency leadership regarding changes in condition, documentation concerns, care coordination needs, and upcoming deadlines.
• Track outstanding clinical documents and follow up with the appropriate parties to ensure timely completion.
• Use the agency's designated mobile application to clock in and out accurately for all scheduled work and visits.
• Attend a required in-person meeting with the CEO once per month to discuss clinical operations, compliance, client needs, and agency priorities.
• Participate in quality assurance and performance improvement activities as assigned.
• Report safety concerns, changes in client condition, suspected abuse or neglect, and other reportable issues according to agency policy and applicable law.
• Perform other RN case management and clinical oversight duties within the scope of practice.

Required Qualifications

• Active, unrestricted Registered Nurse license in Georgia.
• Current CPR/BLS certification.
• Negative TB test
• Understanding of nursing assessments, plans of care, clinical documentation, and supervisory responsibilities.
• Strong written communication, organization, and attention to detail.
• Ability to manage multiple client files, documentation deadlines, and renewal requirements.
• Comfortable using electronic health records, mobile applications, email, and other digital systems.
• Ability to work independently while communicating consistently with agency leadership and staff.
• Reliable transportation and the ability to attend required client visits and monthly in-person meetings.
• Ability to maintain client confidentiality and comply with HIPAA requirements.

Preferred Qualifications

• Previous experience in home health, private-duty nursing, pediatric nursing, or RN case management.
• Familiarity with GAPP, Georgia Medicaid, prior authorizations, or renewal processes.
• Experience reviewing skilled nursing documentation and supervisory visit notes.
• Experience completing admissions, intake assessments, reassessments, and renewal packets.
• Knowledge of pediatric and medically complex client care.
• Familiarity with regulatory compliance and quality assurance in a home-care setting.

Pay: $50.00 - $55.00 per hour
