Job Description

Key Responsibilities:

As a Case Manager, you'll be at the heart of our patient-centered care team, championing collaborative practices that make a real difference in patients' lives. You'll take ownership of the complete care journey—from initial identification and comprehensive assessment through planning, facilitation, and care coordination—ensuring every patient and family receives the support they need to thrive.

Your day-to-day impact includes conducting thorough assessments to understand each patient's unique health care needs, developing personalized care plans that address both clinical and social factors, and facilitating seamless transitions between care settings. You'll navigate complex systems to connect patients with community resources, advocate passionately for your patients' best interests, and coordinate with internal and external stakeholders to optimize outcomes. Your expertise in discharge planning will ensure safe, effective transitions to the next level of care, while your knowledge of preventive guidelines, clinical best practices, and regulatory requirements keeps you ahead of the curve.

You'll evaluate care effectiveness continuously, document meticulously, and leverage your strong problem-solving skills to address complex patient needs simultaneously. By understanding social determinants of health and their profound impact on wellbeing, you'll help bridge gaps in care and promote both patient safety and cost-effective solutions. Qualifications

EDUCATION/EXPERIENCE REQUIRED:

• Bachelor's degree in Nursing OR Master's degree in Social Work
• Minimum 3 years of clinical experience
• Excellent verbal communication and written documentation skills
• Excellent customer service and interpersonal skills with ability to interact with internal and external customers at all organizational levels
• Strong problem-solving, analytical, and decision-making skills
• Strong computer skills and technical knowledge
• Strong organizational, planning, and implementation skills with ability to manage multiple complex patient needs simultaneously
• Knowledge of preventive service guidelines and clinical practice guidelines
• Knowledge of Medicare and Medicaid regulations and case management principles
• Knowledge of medical ethics and legal implications related to case management
• Understanding of social determinants of health and their impact on patient wellbeing
• Demonstrated ability to facilitate community resources to meet the needs of diverse populations
• Strong sense of compassion with ability to successfully advocate for patients and their families

CERTIFICATIONS/LICENSURES REQUIRED:

• Registered Nurse (RN) OR Licensed Social Worker (LMSW) with valid, unrestricted State of Michigan license

PREFERRED QUALIFICATIONS:

• Experience in discharge planning, home health care, rehabilitative medicine, or managed care
• Knowledge of behavior change theory
• Certification in Case Management (CCM) by the Commission for Case Management Certification (CCMC) or Accredited Case Manager (ACM) by the American Case Management Association
