INTRODUCTION
We are seeking a detail-oriented full time Case Manager Assistant to assist the Case Manager and administrative staff.

ROLE AND RESPONSIBILITIES
- Assist with admission of new residents by providing program explanation, housing expectations, and resources.
- Ensure all intake paperwork is complete and accurate and entered into the appropriate systems in conjunction with the program case manager.
- Maintain ongoing communication.
- Process, manage and maintain accurate records and other documentation.
- Assist case manager in preparing for resident meetings, service plan reviews, and follow-up appointments.
- Assist the case manager in gathering documentation required for benefit applications, housing assistance, or other funding and community resource programs.
- Assist the case manager in obtaining authorizations (including prior authorizations and continuing authorizations).
BASIC QUALIFICATIONS
- Associates or higher degree in social work, human services, psychology and/or 2 years’ minimum experience in a healthcare or related field.
- Demonstrated ability to successfully collaborate and work productively as a member of a multidisciplinary team.
- Excellent written and oral communication skills and interpersonal skills, including the ability to work well with a diverse range of individuals.
- Ability to maintain and properly safeguard confidential data and information.
- Must be able to read, write, hear, understand, and speak English, to ensure accurate communication and documentation.
- Ability to work effectively in a busy environment with multiple distractions.
