JOB SUMMARY
The Self-Management Coordinator is responsible for implementing programs and services for the self-managed members in the Client Services Department. The primary purpose of the position is to support and encourage participants to maintain medical adherence and retention to medical care through individual social service interventions and continuing education.
ESSENTIAL DUTIES & RESPONSIBILITIES
- Educate members about self-management program and services, policies, and procedure to ensure that members have clear expectations of services.
- Meet with Self-Managed members for individual consultations to address short-term social service needs and to assist with locating and accessing various community resources.
- Complete Self-Management Care Plan for individuals with service needs.
- Maintain SMP spreadsheet for active Self-management members.
- Advocate and mediate as needed on member's behalf to decrease or eliminate barriers to care.
- Provide active referrals, application assistance, and support to members (ie. referrals for eye exams, referrals for furniture, applications for medication and insurance premium assistance, etc.).
- Facilitate the smooth transfer of self-managed members back into case management when more intensive interventions are needed.
- Contact all members of the Self-Management program as needed and/or at a minimum of once every 6 months to assess continued medical adherence and stability.
- Educate members about the importance of medication adherence and assist members becoming and/or remaining compliance to improve quality of life.
- Compile and create educational information and resource materials in user-friendly formats for distribution to Self-managed members in their welcome packet.
- Maintain Monthly SMP Newsletter with upcoming community and AID Atlanta/AHF events, services, and resources.
- Participate in various trainings held internally and externally ensuring a continuous knowledge base of programs that benefit members.
- Assess, verify, and maintain records of a member's eligibility for Self-Management program.
- Perform timely and accurate input of all required client information in Client Chart (data entry and supporting documentation) to ensure compliance with regulatory and contractual requirements.
- Document all work done with and on behalf of members into agency and/or funder client database system in accordance with documentation standard. (eCW, e2Fulton, etc)
- Abide by NASW code of ethics, HIPAA and Atlanta EMA Case Management Standards to ensure a high level of professionalism is maintained.
- Perform routine self-audits and maintain charts to ensure members information and eligibility documents are current.
- Make referrals appropriate for behavioral health services as per screening tool assessment and other assessments of member needs.
- Effective collaboration with programs within AID Atlanta to ensure appropriate services (i.e. Behavior Health Groups, Intervention Workshops, Nutritionist, etc.).
- Assist SSM with all duties assigned for monthly educational programs.
- Effective collaboration with medical providers to ensure members are receiving medical appropriate services.

QUALIFICATIONS
EDUCATION & WORK EXPERIENCE
- At least 1 year experience in the field of social work/case management or similar field
- Experience in the field of HIV is preferred, but not required.
- Bachelor's degree preferred, High school diploma or equivalent required
- Ability to learn quickly
- Experience in the efficient use of electronic medical records preferred
- Must pass criminal background check
