INTRODUCTION
The Family Practice Nurse Practitioner (FNP) provides comprehensive primary care services to patients of all ages in a community-based, patient-centered environment. Working within the scope of practice and under the collaborative supervision of a physician, the FNP promotes health, prevents illness, manages chronic conditions, and delivers high-quality, culturally competent care. This position is key in fulfilling the FQHC’s mission to improve access to quality health care for underserved populations.
ESSENTIAL DUTIES AND RESPONSIBILITIES:
- Provide comprehensive medical services, including physical examinations, health assessments, diagnosis, and treatment of acute and chronic illnesses.
- Develop and manage patient care plans in collaboration with the patient, family, and multidisciplinary team.
- Prescribe medications and other treatments in accordance with state and federal regulations.
- Order, interpret, and follow up on diagnostic tests and laboratory results.
- Deliver preventive care including immunizations, screenings, and education on wellness and disease prevention.
- Coordinate care with internal departments (behavioral health, dental, outreach) and external agencies to ensure continuity of care.
- Participate in quality improvement initiatives and population health management strategies.
- Document all patient encounters accurately and timely in the electronic health record (EHR).
- Serve as a preceptor or mentor to students or new providers when appropriate.
- Participate in staff meetings, provider meetings, and continuing education programs.
- Comply with all FQHC policies, procedures, and standards, including those related to compliance, safety, and infection control.
- Support and participate in UDS reporting, PCMH recognition, and other grant or program requirements.
QUALIFICATIONS
Education and Experience:
- Master of Science in Nursing (MSN) or Doctor of Nursing Practice (DNP).
- Minimum of 1-2 years’ experience in primary care or family practice preferred; new graduates with strong clinical rotations may be considered.
Licensure and Certification:
- Current, unrestricted license to practice as an Advanced Registered Nurse Practitioner (ARNP) in the state of Iowa.
- Certification as a Family Nurse Practitioner by AANP or ANCC.
- DEA registration and state prescriptive authority required.
- BLS certification.
KNOWLEDGE, SKILLS, AND ABILITIES:
- Knowledge of primary care principles, chronic disease management, and community health.
- Strong communication skills with the ability to build rapport with diverse patient populations.
- Commitment to patient-centered, trauma-informed, and culturally competent care.
- Proficiency with EHR systems; experience with Epic a plus.
- Ability to work independently and collaboratively within a multidisciplinary team.
- Bilingual skills preferred but not required.
WORK ENVIRONMENT:
- Clinical environment in an outpatient community health center.
- May require occasional evening hours to meet patient needs.
Physical Requirements:
- Ability to sit, stand, walk, and perform repetitive motions for extended periods.
- Occasional lifting of up to 25 pounds may be required.
