General Purpose:
The Psychiatrist is expected to work closely with other members of the Health Care Team to provide quality, evidence-based psychiatric care to patients across the lifespan in an outpatient setting. The position emphasizes collaborative, integrated care within the Behavioral Health department and with primary care and medical staff throughout the organization.
Description of Duties:
Clinical Evaluation and Treatment
- Provides comprehensive psychiatric evaluations to individuals, families, and groups, assessing mental health needs across the lifespan (children, adolescents, adults, and older adults).
- Offers comprehensive psychiatric services to the mental health population, conducting thorough assessments, establishing diagnoses, and treating individuals and families with psychiatric disorders or the potential for such disorders using the full scope of psychiatric expertise, including prescription of medication, medication management, administration of psychotherapy, and management of complex psychiatric and medical comorbidities.
- Develops and maintains individualized treatment plans with measurable therapeutic goals in collaboration with patients, families/guardians (when appropriate), and the multidisciplinary care team.
- Provides high quality, patient-centered psychiatric care with maximum patient comfort and safety in both in-person and virtual care settings, in accordance with organizational telemedicine policies and applicable state and federal regulations.
- Provides culturally competent care to diverse and underserved populations.
- Carries a panel of patients for ongoing care, monitoring, and clinical evaluations.
- Meets regularly with patients to establish diagnosis of mental disorders, assess response to treatment, and adjust treatment plans accordingly.
- Prescribes psychotropic medications and/or recommends evidence-based psychotherapeutic interventions according to the patient's clinical needs and preferences.
Risk Assessment and Safety
- Conducts comprehensive risk assessments including suicide, self-harm, violence, and abuse/neglect potential at initial evaluation and ongoing visits, in accordance with organizational protocols and clinical guidelines.
- Documents and implements evidence-based safety plans when indicated, involving appropriate family members, supports, and community resources.
- Arranges appropriate higher levels of care (emergency department, inpatient hospitalization, crisis services) when clinically indicated and ensures continuity of care following acute episodes or hospitalizations.
- Responds to urgent clinical situations within the clinic setting, including same-day urgent psychiatric assessments as needed.
Interdisciplinary and Collaborative Care
- Participates actively in the interdisciplinary Behavioral Health team, collaborating with psychiatrists, psychiatric nurse practitioners, behavioral health therapists, and case managers regarding patient diagnosis, treatment planning, and care coordination to support patient care.
- Participates in weekly high-risk case review meetings for the assigned office location, presenting and discussing complex, high-acuity, or high-risk cases in an interdisciplinary setting and implementing agreed-upon safety and treatment recommendations.
- Provides clinical consultation and guidance to primary care providers and medical staff regarding the diagnosis and management of common mental health conditions in patients they serve.
- Offers expertise and recommendations regarding psychotropic prescribing, monitoring for medication side effects, and clinical decision-making for appropriate specialist referrals, in support of the Collaborative Care Model.
- Coordinates care with external providers and agencies (schools, inpatient psychiatric units, community mental health organizations, etc.) as appropriate, with proper patient consent and release of information.
- Supports integrated care initiatives within the health center, including warm handoffs, shared care planning, and coordination between medical and behavioral health services.
Care Coordination and Continuity
- Ensures appropriate follow-up care following psychiatric hospitalizations, emergency department visits, or crisis episodes.
- Provides timely, clear documentation and communication to referring providers regarding patient diagnoses, treatment plans, psychotropic medication changes, and clinical recommendations.
- Completes all patient medical and administrative forms (disability, medical clearance, school documentation, etc.) on a timely basis.
Telehealth Services
- Provides psychiatric evaluation and ongoing treatment via approved telehealth platforms when clinically appropriate and in accordance with organizational policy.
- Ensures patient privacy and confidentiality in all telehealth encounters and verifies patient identity and geographic location per regulatory requirements.
- Documents telehealth visits thoroughly in the electronic health record per organizational standards and applicable regulatory and payer requirements.
Quality Assurance, Documentation, and Compliance
- Maintains timely, accurate, and complete documentation in the electronic health record (ECW), consistent with Cornerstone standards, payer requirements, and regulatory guidelines, including proper coding and billing documentation.
- Participates actively in Cornerstone Quality Assurance and Quality Improvement programs, including utilization of evidence-based screening and outcome measurement tools (e.g., PHQ-9, GAD-7, Columbia-Suicide Severity Rating Scale) to achieve organizational goals and improve patient outcomes.
- Meets defined productivity expectations and panel management goals while maintaining quality of clinical care and patient safety.
- Maintains knowledge of and adheres to all clinical, administrative, and general policies and procedures outlined in the Cornerstone Policy and Procedure Manual and Employee Handbook.
- Maintains confidentiality of all aspects of Cornerstone, including but not limited to patient confidentiality, financial information, and employee relations.
Professional Development and Leadership
- Provides clinical consultation to psychiatric nurse practitioners and behavioral health therapists regarding complex diagnostic formulations and medication management questions.
- Provides leadership opportunities for students, interns, and/or new hires by offering supervision or shadowing opportunities as they arise.
- Attends and actively participates in monthly departmental meetings, all-staff meetings, and other required organizational meetings.
- Maintains current knowledge of evidence-based psychiatric practice, clinical guidelines, and best practices in outpatient psychiatry.
- Performs other clinically or administratively relevant duties as assigned by the Chief of Psychiatry or Chief Medical Officer.
Requirements
- Graduation from an accredited medical school with a New York State license to practice medicine
- Must be Board eligible with the expectation that will become Board Certified in given specialty within five (5) years of hire
