Eliot is seeking an Intensive Case Manager to join our Adult Community Clinical Services (ACCS) team. Adult Community Clinical Services (ACCS) provides clinically focused evidenced based interventions to adults with severe and persistent mental illness. Services are person centered and are delivered by a multidisciplinary team of professionals in either residential or community based settings. In this role you will provide side by side interventions to clients that focus on problem solving, skills training, modeling behavior, optimism and encouragement.
Responsibilities:
- Provide community outreach to each individual as part of the multi-disciplinary team.
- Offer side-by-side interventions that focus on problem-solving, skill training, modeling, optimism, and encouragement.
- Collaborate with the team and each individual to develop a person-centered treatment plan.
- Support individuals in becoming fully integrated into their communities by helping them build natural supports.
- Empower individuals to take control of their lives, focusing on their treatment, leisure activities, employment, and personal recovery.
- Provide on-call coverage, as part of a rotation, during evenings and weekends.
Qualifications
- Bachelor's or Master's degree in Psychology, Social Work, or related field
- Experience working with adults with mental illness
- Valid Massachusetts drivers license
Schedule:
- Monday-Friday 8am-4pm (with rotating weekend and evening shifts)
Bilingual candidates fluent in a needed language may be eligible for a 6% salary increase.
