ROLE AND RESPONSIBILITIES
As a PACE Personal Care Attendant, applicants have the opportunity to play an active, mission-driven role focused on improving the health and quality of life for older adults participating in the PACE Program. This position is essential in supporting participants in the PACE program by providing personal care and activities to enhance their quality of life while promoting independence.
Essential Responsibilities:
- Assist participants with activities of daily living, including personal hygiene, grooming, and dressing.
- Collaborate in the development and implementation of participant care plans as requested.
- Engage with participants to provide emotional support, facilitate meaningful conversations, and assist with socialization activities at the Center.
- Aid participants with mobility, safe transfers, and the use of assistive devices within the PACE Center.
- Support the transportation team to ensure participant safety during van loading and unloading.
- Accompany participants during van rides and at external appointments as indicated by their care plan.
- Work closely with the kitchen team to assist participants during meals and snacks, following dietary orders specified in the plan of care.
- Report any changes in participants’ physical or emotional condition to the care team.
- Assist in collecting vital signs data such as weights, blood pressures, and temperatures.
- Maintain accurate records of activities, participant progress, and any changes in participants’ conditions.
- Serve on various internal and external committees as required or designated.
- Participate in team meetings, caregiver groups, and special events at the PACE Center as needed.
- Provide occasional coverage as a Personal Care Attendant (PCA) to participants in their homes.
- Foster a safe and supportive environment conducive to the professional development of healthcare professionals.
Supporting Responsibilities:
- Meet department and company performance and attendance expectations.
- Follow privacy policy and HIPAA laws and regulations concerning confidentiality and security of protected health information.
- Perform other duties as assigned.
BASIC QUALIFICATIONS
Work Experience: A minimum of 1 year of experience working with elderly or frail populations is required. Previous experience as an Activities Aide or Assistant is preferred.
Education, Certificates, Licenses: A high school diploma or equivalent is required. Certified Nursing Assistant (CNA) certification is preferred. CPR and First Aid certifications are required. A Food Handler’s Card or applicable sanitation certification is required at the time of hire or within 30 days of hire. Valid Oregon Driver’s License is required.
Knowledge:
Strong interpersonal and communication skills, with a friendly and approachable demeanor. Excellent organizational skills and the ability to manage multiple tasks effectively. Ability to work collaboratively within a team environment. Proficiency in using computers and various software applications, including Word, Excel, PowerPoint, and Outlook. Ability to work independently with minimal supervision.
Each member of the PACE organization's staff that has direct contact with participants must meet the following conditions:
- Be legally authorized (for example, currently licensed, registered or certified if applicable) to practice in the State in which he or she performs the function or action.
- Only act within the scope of his or her authority to practice.
- Have 1 year of experience working with a frail or elderly population.
