INTRODUCTION
Will provide clinic- and field-based health education and skill building services to the patients. Accompanies patients to primary care appointments and other social service encounters as warranted. Coordinates ongoing logistical support for appointment keeping, reminders, transportation and childcare arrangements. Responsible for tracking the patient's progress through the health education curriculum.
ROLE AND RESPONSIBILITIES

- Communication skills necessary to interpret age specific response to treatment.
- Knowledge of appropriate resources.
- Utilizes resources appropriate to patient needs.
- Ability to prioritize; manage and work independently on caseloads.
- Assessment of pertinent psycho-social factors.
- Positive and respectful interaction with patients, co-workers, providers and outside providers.
- Ability to problem solve outside routine area of coverage.
- Provides coaching to patients.
- Effective use of self and knowledge of appropriate resources.
- Conducts patient intakes and follow up progress notes and documents encounters in appropriate Retention Adherence Program (RAP) records and databases.
- Conducts treatment adherence education with the patients, identification of barriers to adherence and documents all encounters in chart.
- Maintains active caseload of assigned clients and completes needs assessment, client service plans, reassessments, progress notes and follow-up call/letter in a timely manner.
- Responsible for entering patient encounters and service delivery data into program-specific databases in a timely manner.
- Coordinates scheduling/follow-up appointments and referrals.
- Works with ID medical provider to identify patients that need treatment adherence.
- Ability to assess patients' insurance needs and assist with enrolling the patient with insurance/benefits services (ADAP, HASA, Medicaid).
- Provide case management services (caseload of 35+ HIV/AIDS adults).
- Coordinates all medical care and service for assigned caseload and identifies and addresses barriers to adherence and engagement in care.
- Actively participates in weekly interdisciplinary case conferences and documents the outcome of the meeting in the patients' charts.
- Uses evidence-based interventions such as Motivational Interviewing, DEBI's, etc. to motivate positive change in patient behavior.
- Other duties as assigned.
BASIC QUALIFICATIONS
- This position requires a minimum formal education of Associate Degree or certification equivalent and a minimum of 3 years job related experience.
- Desired (not required) criteria include: A minimum plus 2-3 years experience in HIV and medical coordination.
- Should have strong socio-cultural identification with target population.
- Must strongly discourage the hiring of actively enrolled patients from the program or partner medical provider to protect patient confidentiality.
