ROLE AND RESPONSIBILITIES
Under the direction of the Manager, Diabetes Education Program, the Diabetes Educator will be responsible for the assessment, planning, implementation, and evaluation of Diabetes Self-Management Education and Support plans (DSMES) for patients and families with diabetes. Collaborates with PCPs and/or diabetes clinicians to develop and implement diabetes self-management education plans for patients with diabetes.
- Utilizes patient education principles to assess, plan, implement, and evaluate comprehensive DSMES plans for patients with diabetes.
- Conducts an assessment of the patient’s knowledge base of diabetes and readiness and motivation to learn. Identify any barriers to learning including age, physical, cognitive, environmental, cultural, language, financial, psychological, and spiritual needs.
- Based upon assessment, design an individualized DSMES plan based on the patient and multi-disciplinary teams mutually agreed upon goals.
- Promotes continuity of care by working collaboratively with all members of the multi-disciplinary team in helping patients achieve diabetes goals.
- Utilizes adult teaching-learning methods and a variety of presentation styles to provide education to patients and families.
POSITION QUALIFICATIONS
License/Certification/Education:
Required:
1. Bachelor Degree from Accreditation Council for Education in Nutrition and Dietetics (ACEND) approved four-year program in Food and Nutrition or related field.
2. Certified Diabetes Educator (CDE) credential required within two years of hire.
3. Maintains a license with the Board of Dietitians and Nutritionists in the Commonwealth of Massachusetts.
4. Maintains registration with the Commission on Dietetic Registration.
Preferred:
1. Master’s degree.
Experience/Skills:
Required:
1. Minimum of two years diabetes related experience.
2. Ability to influence the quality of patient care in an advisory capacity to the physician and other health care team members.
3. Demonstrated skill in analyzing problems and making decisions.
4. Oral and written communication skills are essential to interact with patients, staff, and referring providers.
Preferred:
1. Diabetes Education Experience in outpatient setting.
2. Experience with EMR.
Unless certification, licensure, or registration is required, an equivalent combination of education and experience which provides proficiency in the areas of responsibility listed in this description may be substituted for the above requirements.
Department-specific competencies and their measurements will be developed and maintained in the individual departments. The competencies will be maintained and attached to the departmental job description. Responsible managers will review competencies with position incumbents.

Work is considered sedentary. Position requires work indoors in a patient environment.
