Verify your NCLEX and licensure transfer eligibility
U.S. states require foreign-trained RNs to pass the NCLEX before practicing. Start the CGFNS credentials evaluation and your target state's nursing board application early, as processing can take several months and host employers will ask for proof of eligibility.
Target outpatient settings with established exchange programs
Ambulatory care networks, federally qualified health centers, and academic medical system clinics are more likely to have existing relationships with J-1 designated sponsors. Prioritize host employers whose HR teams already understand the DS-2019 issuance and training plan requirements.
Search J-1-aligned roles on Migrate Mate
Use Migrate Mate to filter for outpatient RN positions at host employers open to exchange visitor arrangements. It surfaces roles where the hiring organization has prior experience working alongside J-1 designated sponsors, saving you significant outreach time.
Prepare a structured training plan before outreach
Your designated sponsor will require a detailed Training Plan form outlining specific clinical competencies, supervision arrangements, and program duration. Drafting this in advance with your prospective host employer signals readiness and accelerates the DS-2019 application process considerably.
Clarify the two-year home residency rule upfront
Many outpatient RN exchange programs are subject to the two-year home-country physical presence requirement. Confirm whether your program triggers this rule before accepting an offer, since it affects future visa options including H-1B and green card petitions.
Confirm host employer compliance obligations before signing
The host facility must maintain workers' compensation coverage, provide suitable supervision by a licensed U.S. RN, and cooperate with your designated sponsor's monitoring visits. Ask HR to confirm these obligations in writing before your DS-2019 application is submitted.

Find Outpatient Registered Nurse JobsOutpatient Registered Nurse J-1 Visa: Frequently Asked Questions
Which J-1 program category fits an outpatient registered nurse?
Most outpatient RNs enter under the Trainee or Specialist category. The Trainee category suits nurses who graduated within the past 12 months or have fewer than five years of post-degree experience. The Specialist category fits experienced RNs bringing a demonstrable specialty, such as chronic disease management or infusion therapy, to a U.S. host facility.
Who actually sponsors the J-1 visa for an outpatient RN position?
The visa sponsor is a U.S. Department of State-designated organization, not the hospital or clinic hiring you. Organizations such as CIEE or Cultural Vistas issue the DS-2019 form that authorizes your exchange visitor status. The outpatient facility is your host employer, a separate role from the designated sponsor that administers your program.
How do I find outpatient employers willing to host a J-1 exchange visitor?
Search Migrate Mate to identify outpatient RN roles at employers that have experience hosting exchange visitors. Many ambulatory care networks and academic health system clinics are open to J-1 arrangements but don't advertise it explicitly. Filtering for these roles first saves you time compared to cold outreach to employers unfamiliar with the DS-2019 process.
Does the two-year home residency requirement apply to outpatient RN exchange programs?
It often does, particularly when government funding, the Conrad State 30 waiver program for medically underserved areas, or graduate medical education is involved. Whether it applies depends on your country of nationality, the funding source of your program, and your specific J-1 category. Confirm this with your designated sponsor before accepting any host employer offer.
Can I transition from J-1 status to an H-1B or permanent residency after my outpatient RN program?
If the two-year home residency requirement applies to you, you must fulfill it or obtain a waiver before changing to H-1B or adjusting status. Outpatient RNs in medically underserved areas may qualify for a Conrad State 30 waiver, which satisfies the requirement in exchange for a service commitment. An immigration attorney should review your DS-2019 and program details before you plan any status change.
