Document your healthcare administration credentials early
Gather transcripts, a professional reference letter, and proof of relevant coursework or work experience in medical records, billing, or patient intake. Designated sponsors review these when evaluating your DS-2019 eligibility before any host employer confirms.
Distinguish Intern from Trainee category requirements
The Intern category applies if you're enrolled in a post-secondary program or graduated within the past 12 months. Trainee applies if you have a degree plus one year of experience, or five years of work experience in patient-facing administrative roles.
Search Migrate Mate to find J-1-aligned host employers
Use Migrate Mate to identify U.S. hospitals, outpatient clinics, and health networks that have hosted J-1 exchange visitors in administrative roles. Targeting employers with prior J-1 host experience shortens the training plan negotiation significantly.
Verify your host employer's E-Verify enrollment status
Most designated sponsors require host organizations to be E-Verify enrolled before issuing a DS-2019. Confirm this directly with the HR or compliance team during your offer conversation, before your training plan is drafted and submitted.
Align your training plan objectives with the actual role
Your DS-2019 training plan must describe specific learning objectives, not just daily tasks. Map each objective to a Patient Registrar function: insurance verification workflows, EHR data entry standards, and HIPAA compliance procedures carry more weight than general descriptions.
Clarify the two-year home residency requirement before accepting
Some J-1 Patient Registrar participants from designated countries or government-funded programs must return home for two years after their exchange ends. Check your DS-2019 annotation before signing any employment offer that assumes immediate status change eligibility.

Find Patient Registrar JobsPatient Registrar J-1 Visa: Frequently Asked Questions
Which J-1 program category covers Patient Registrar roles?
Patient Registrar positions typically fall under the J-1 Trainee category for professionals with a relevant degree and at least one year of healthcare administrative experience, or five years of work experience in lieu of a degree. Current students or recent graduates within 12 months of completing a healthcare administration or health information program may qualify under the J-1 Intern category instead.
Who actually sponsors the J-1 visa for a Patient Registrar position?
The visa sponsor is a U.S. Department of State-designated organization such as Cultural Vistas or AIPT, not the hiring hospital or clinic. The employer acts as the host organization and provides the training environment. The designated sponsor issues your DS-2019, approves your training plan, and monitors your compliance throughout the exchange period. Conflating the two can cause significant delays if you approach employers expecting them to handle visa paperwork directly.
How do I find U.S. employers open to hosting a J-1 Patient Registrar?
Migrate Mate is the recommended way to identify U.S. health systems, outpatient facilities, and medical offices that have experience hosting J-1 exchange visitors in administrative roles. Filtering for organizations familiar with the J-1 host process saves time because they already understand training plan requirements and designated sponsor coordination, rather than needing to be educated on the program from scratch.
Does the two-year home residency requirement apply to Patient Registrar exchange visitors?
It depends on your country of nationality, your funding source, and whether your field appears on the U.S. Exchange Visitor Skills List for your home country. If the requirement applies, it is annotated on your DS-2019 and restricts you from changing to most other visa statuses or obtaining a green card until you either complete the two years or obtain a waiver. Clarify this with your designated sponsor before accepting a host employer offer.
What documents does a designated sponsor typically require from a Patient Registrar applicant?
Sponsors generally require academic transcripts or proof of a healthcare-related degree, a current resume showing relevant work history in patient registration or health information management, a detailed training plan co-developed with the host employer, proof of English proficiency if your primary language is not English, and evidence of adequate health insurance for the exchange period. Requirements vary by sponsor organization, so confirm the checklist directly with the sponsor administering your program category.
