Verify your credentials meet U.S. nursing standards
Before targeting host employers, confirm your nursing license is eligible for U.S. endorsement through your state's Board of Nursing and that your NCLEX-RN results are ready. Many host facilities require credential verification through CGFNS before extending an offer.
Distinguish Trainee from Specialist category eligibility
If you graduated within the last year and have limited post-degree experience, the J-1 Trainee category applies. Nurses with a defined specialty and substantial clinical experience typically qualify under Specialist, which changes how your training plan is structured and reviewed by the designated sponsor.
Search Migrate Mate to find J-1-aligned host employers
Use Migrate Mate to identify U.S. hospitals, health systems, and staffing agencies that have hosted J-1 exchange visitors in nursing roles. Filtering by host employer history saves time compared to cold-contacting facilities that have no J-1 infrastructure in place.
Confirm the host facility has an active sponsor relationship
Not every hospital can host a J-1 nurse. The facility must have an existing agreement with a State Department-designated sponsor organization. Ask your prospective host HR team directly which sponsor they work with before advancing the offer conversation.
Review the 2-year home residency requirement early
Many J-1 Trainee and Specialist participants are subject to a two-year home-country residency requirement after their program ends. This affects eligibility for H-1B and immigrant visas afterward, so factor it into your long-term U.S. career planning before accepting a J-1 placement.
Secure an OET or IELTS score before the DS-2019 is issued
Designated sponsor organizations require English proficiency documentation before issuing a DS-2019. Accepted tests include OET and IELTS. Delays in submitting scores are a common cause of DS-2019 processing holdups that push back your program start date.

Find Travel Nurse RN JobsTravel Nurse RN J-1 Visa: Frequently Asked Questions
Which J-1 program category applies to Travel Nurse RN roles?
Most Travel Nurse RNs enter under the J-1 Trainee or Specialist category, depending on experience level. Trainee is for nurses within 12 months of graduation or those with fewer than one year of post-degree experience. Specialist applies to experienced nurses with a defined clinical focus. Neither category covers permanent clinical employment; both require a structured training or exchange program plan.
Who actually sponsors a J-1 visa for a travel nurse, the hospital or a separate organization?
The visa sponsor is a U.S. Department of State-designated organization, not the hospital itself. Organizations such as IIE, AIPT, or Cultural Vistas issue the DS-2019 form and monitor program compliance. The hospital or staffing agency is the host employer. You need both: a host willing to take you on and a designated sponsor willing to administer your exchange program.
How do I find U.S. hospitals that have hosted J-1 exchange nurses before?
Searching Migrate Mate lets you identify health systems and facilities that have experience hosting J-1 exchange visitors in nursing roles. Targeting employers with existing J-1 infrastructure is more efficient than approaching facilities with no sponsor relationship in place, since those hosts will need to establish one before your DS-2019 can be issued.
Can I switch to an H-1B or green card after my J-1 travel nurse program ends?
Only if the two-year home residency requirement does not apply to you, or if you obtain a waiver. J-1 Trainee and Specialist participants from countries with exchange visitor agreements are often subject to this requirement, which bars H-1B and immigrant visa eligibility until it is fulfilled or waived. USCIS and the State Department both enforce this condition, so confirm your status with your designated sponsor before making long-term plans.
What documents does a designated sponsor need before issuing a DS-2019 for a travel nurse?
Designated sponsors typically require your nursing degree and transcript, NCLEX-RN results, English proficiency test scores (OET or IELTS are commonly accepted), a completed training plan agreed upon with the host facility, proof of health insurance meeting J-1 minimum standards, and a signed host organization agreement. Missing any of these documents is the most common reason DS-2019 issuance is delayed.
