Get Access To All JobsTips for Finding J-1 Visa Sponsorship as an Internal Medicine Physician
Verify your ECFMG certification before applying
ECFMG certification is a hard prerequisite for J-1 Physician sponsorship. Confirm your USMLE scores, medical credentials, and English proficiency documentation are current before approaching any host institution, as incomplete files stall the DS-2019 issuance process.
Identify programs with J-1 waiver pathways early
Many Internal Medicine residency and fellowship programs sponsor J-1 Physician status but differ on whether they support Conrad 30 or federal agency waivers afterward. Ask programs explicitly which waiver types they have historically supported for graduates facing the two-year home residency requirement.
Search employers already hosting J-1 physicians on Migrate Mate
Use Migrate Mate to filter Internal Medicine Physician roles by employers with active J-1 hosting history, so you target health systems and teaching hospitals already familiar with the DS-2019 process rather than educating a new employer from scratch.
Confirm the host institution's ACGME accreditation status
ECFMG will only issue a DS-2019 for training at ACGME-accredited programs. Before accepting an offer, verify the program's accreditation status directly through ACGME's public database, since a lapse mid-program can terminate your J-1 status.
Request a written 2-year requirement determination upfront
Not every Internal Medicine J-1 Physician is automatically subject to the two-year home residency requirement, but most are. Ask ECFMG for a written determination specific to your country of nationality and country of medical training before signing your training contract.
Align your DS-2019 program dates with licensing timelines
State medical boards require J-1 Physician trainees to hold a valid DS-2019 covering the entire licensure period. Coordinate with your ECFMG sponsor well before renewal deadlines to avoid gaps that would interrupt your state license and hospital privileges.

Find Internal Medicine Physician JobsInternal Medicine Physician J-1 Visa: Frequently Asked Questions
Which J-1 program category applies to Internal Medicine Physicians?
Internal Medicine Physicians training in residency or fellowship programs use the J-1 Physician category, administered exclusively through ECFMG as the sole U.S. Department of State-designated sponsor for this category. The Physician category is distinct from Intern, Trainee, or Research Scholar and is specifically designed for graduate medical education at ACGME-accredited institutions.
What is the two-year home residency requirement, and does it apply to me?
The two-year home residency requirement under INA Section 212(e) obligates certain J-1 Physicians to return to their home country for two years before changing to most other visa statuses. It applies if your training was funded by your home government or if your nationality is on the State Department's Exchange Visitor Skills List. ECFMG can issue a written determination specific to your circumstances.
Who actually sponsors the J-1 visa for an Internal Medicine Physician position?
ECFMG is the designated sponsor and issues the DS-2019, while the hiring hospital or health system serves as the host institution. The employer does not sponsor the J-1 visa directly. This three-party structure means you must be accepted by both a qualifying ACGME-accredited training program and have your paperwork processed through ECFMG before your visa is issued.
How do I find Internal Medicine Physician positions where the host employer is already familiar with J-1 requirements?
Use Migrate Mate to search for Internal Medicine Physician roles filtered by employers with demonstrated J-1 hosting experience. Targeting health systems that have previously hosted J-1 Physicians shortens the negotiation process because their graduate medical education and legal teams already understand DS-2019 timelines, ECFMG compliance requirements, and training plan documentation.
Can I apply for a J-1 waiver after completing my Internal Medicine training?
Yes. Physicians subject to the two-year home residency requirement can apply for a waiver through several federal agency programs, including the Conrad 30 program administered by state health departments, which requires a three-year service commitment in an underserved area. Waiver type availability depends on your specialty, your state, and whether your training program has historically supported departing residents through the process.
