Align your credentials with specialty occupation standards
Regional Medical Director roles require demonstrated clinical leadership and administrative authority. Gather board certification documentation, letters confirming supervisory scope, and a CV that distinguishes executive oversight from direct patient care before approaching host employers.
Identify host employers with existing sponsor relationships
Hospital systems and regional health networks that have previously hosted J-1 physicians are already familiar with DS-2019 obligations. Search Migrate Mate to find Regional Medical Director openings at organizations with documented J-1 hosting experience.
Clarify the two-year home residency requirement early
Physician-category J-1 holders whose training was government-funded or who come from countries on the Exchange Visitor Skills List are subject to a two-year home residency requirement before changing status or applying for most immigrant visas. Confirm your eligibility for a waiver before accepting an offer.
Verify the designated sponsor covers your program category
ECFMG sponsors J-1 physicians in clinical training, but a Regional Medical Director role with primarily administrative duties may fall under the Research Scholar or Specialist category, requiring a different designated sponsor such as IIE or Cultural Vistas. Confirm the category match before any employer initiates the DS-2019 request.
Request a written training plan before the DS-2019 is filed
Designated sponsors require a detailed training plan for Trainee and Research Scholar categories. For a medical director role, document the specific competencies, reporting structures, and program objectives the host employer will support throughout your exchange period.
Confirm the host employer understands its obligations
The host organization must provide a worksite that meets USIA and Department of State standards, maintain contact with the designated sponsor, and report any material changes to your program. Ask the employer to confirm they have completed prior DS-2019 hosting agreements before you sign an offer.

Find Regional Medical Director JobsRegional Medical Director J-1 Visa: Frequently Asked Questions
Which J-1 program category applies to a Regional Medical Director role?
It depends on the role's primary function. If the position involves clinical training or graduate medical education, the Physician category administered by ECFMG typically applies. If the role is primarily administrative or focused on health systems research, the Research Scholar or Specialist category is more appropriate, with a different designated sponsor organization issuing the DS-2019.
Who actually sponsors the J-1 visa for this role, the hospital or a separate organization?
The hiring hospital or health system is the host employer, not the visa sponsor. The J-1 sponsor is a U.S. Department of State-designated organization, such as ECFMG, IIE, or Cultural Vistas, that issues the DS-2019 form, monitors your program compliance, and serves as the official point of contact with the State Department throughout your exchange period.
How can I find Regional Medical Director positions where the host employer is open to J-1 exchange visitors?
Most standard job boards don't filter by visa sponsorship category. Migrate Mate lets you search Regional Medical Director roles specifically at U.S. employers that align with J-1 hosting arrangements, so you're not cold-applying to organizations unfamiliar with the DS-2019 process or the designated sponsor relationship.
Does the two-year home residency requirement affect Regional Medical Director candidates?
It can. Physician-category J-1 holders are subject to the two-year home residency requirement if their home country appears on the Exchange Visitor Skills List or if their medical training received government funding. This requirement must be fulfilled or formally waived before you can apply for an H-1B, L-1, or most immigrant visas. Confirm your status before accepting an offer.
Can a Regional Medical Director role qualify as a specialty occupation for J-1 purposes?
The J-1 visa does not use the specialty occupation standard that applies to H-1B visas. Instead, the designated sponsor evaluates whether the role provides a genuine exchange experience consistent with the declared program category. For medical director positions, sponsors will assess whether the role involves substantive clinical leadership, research, or training rather than routine administrative functions.
