As our Patient Logistics RN, you will lead transfer center operations for Dignity Health facilities, coordinating complex patient movements so they can receive timely, life-saving care in the appropriate clinical setting.
Every day you will oversee shift operations and serve as the primary point of contact for hospital administration, nursing leadership, medical staff, and transportation vendors. You will be expected to facilitate efficient patient bed assignments, manage critical resources, and uphold strict adherence to organizational work-aids and clinical algorithms to ensure seamless patient flow.
To be successful in this role, you must possess strong clinical judgment and leadership experience, with the ability to independently prioritize patient transfers based on medical urgency and resource availability while maintaining rigorous quality assurance standards.
Position Summary
Responsibilities and Essential Functions

Supervision & Leadership:
- Provide supervisory support to Patient Logistics RNs and other staff, holding team members accountable to established standards.
- Manage Transfer Center calls in accordance with established work-aids and algorithms.
- Function and lead independently with high levels of self-initiative and discipline.
- Participate in the orientation of new staff and serve as a resource for hospital staff and vendors.

Operational Excellence:
- Determine facility designations based on diagnoses, planned procedures, and level of care requirements.
- Utilize nursing knowledge to prioritize patient transfers based on the urgency of the condition and bed availability.
- Respond efficiently during emergency situations to ensure the safe and appropriate transfer of all patients.
- Maintain constant communication with internal and external sources to optimize patient flow.

Quality Assurance & Data Management:
- Compile data to ensure compliance with organizational policies, work-aids, and algorithms.
- Perform quality assurance reviews and communicate findings, issues, and barriers to the Director/Manager.
- Support leadership in the preparation of reports, data collection, and analysis for program improvement.
- Ensure accurate and timely distribution of daily reporting.

Clinical Standards & Safety:
- Perform legible, timely, and accurate documentation in compliance with all policies and procedures.
- Apply safety precautions and National Patient Safety Goals to all transfer activities.
- Communicate clearly using accurate medical terminology and report changes in patient conditions to appropriate personnel.
- Maintain a safe working environment and demonstrate accountability for all decisions and behaviors.

Professional Development:
- Participate in service line growth, quality initiatives, and patient satisfaction efforts.
- Continue education by attending non-mandatory offerings relevant to the practice area.
- Set and pursue measurable written goals for professional growth during the annual appraisal process.
- Maintain awareness of current issues and trends within the nursing profession.
Job Requirements
- Current RN license
- Proven proficiency with all Microsoft Office applications
- Familiarity with TeleTracking, Cerner and Midas
- Excellent verbal and electronic communication stills, including professional telephone and email etiquette
- RN with 3-years of clinical experience, one to include acute hospital
- Must have the ability to communicate effectively with a variety of customers including patients, providers, hospital and office staff, health plans, medical groups, internal departments, community resources, and peers
- Ability to work independently and apply critical thinking skills toward making appropriate decisions, as well as in a team environment
- Ability to multi-task and prioritize
- Detail oriented
- Excellent organizational and communication skills and ability to meet timeframes
- Ability to learn quickly and retain complex information
- Travel as needed
- Ability to lift a minimum of 20 pounds
- Progressive responsibilities leading up to or including supervisory role
- Experience in Utilization Management, Case Management or Care Coordination, Managed Care
- ICU RN, Emergency Room RN or Level One Trauma RN
