INTRODUCTION
Valley Professionals Community Health Center is hiring a Licensed Clinical Social Worker, Licensed Mental Health Counselor or a Licensed Clinical Addictions Counselor for our Parke and Vermillion mobile-based team.

- LCSW/LMHC: Base salary of $71,838-$93,470

JOB DUTIES
This position would be responsible for providing professional behavioral healthcare to VPCHC patients and maintaining the related patient documentation. In addition, this person would coordinate patient treatment with a multidisciplinary team to assure continuity of care and the comprehensiveness of health care plans.
REQUIREMENTS
- Current Indiana LSW, LCSW, LMHC, LMHCA, LCAC licensure
- Previous background and training in addictions counseling preferred

- Monday: 8A-5P South Vermillion MS
- Tuesday: Admin or 1/2 day
- Wednesday: Admin or 1/2 day
- Thursday: 8A-5P Rockville Elementary
- Friday: 8A-5P North Vermillion Jr/Sr HS
