DETAILED JOB DESCRIPTION
- Assess the patient’s non-clinical needs and assist accordingly with improving adherence to patient plan of care, including the handling of travel, connecting the patient with community resources and support groups, acquiring insurance and financial information for support of care, etc.
- Orient new patients to the service line through the provision of information about availability of parking, way finding to check-in areas, meeting different members of the care team, etc.
- Handle patient inquiries and collaborating successfully with other members of the care team to assist as needed.
- Facilitate the patient’s transition from the setting of care to the next setting of care.
- Serve as an empathic advocate for the patient throughout the patient’s care.
- May complete unit/department specific duties and expectations as outlined in department documents.
REQUIRED QUALIFICATIONS
Two (2) years of experience in health care, or education may be used in lieu of experience.
PREFERRED QUALIFICATIONS
- Associate's degree in health-related field.
- Additional license/certification requirements as determined by the hiring department.
