Advertised Job Summary
The Advanced Practice Provider(APP) plays a pivotal role in coordinating the continuum of care and managing the overall patient experience within the USC Brain Health Network (UBHN), ensuring good communication, efficient and patient-centered operations, and effective service delivery for every patient. They are responsible for coordinating and navigating the care for all patients referred to the UBHN for services between the primary care provider and referrals to ancillary services and specialties. To demonstrate commitment to conducting business lawfully and ethically by consistently adhering to compliance policies, procedures and regulations. This position will travel across the state of South Carolina.
Job Related Minimum Required Education and Experience
* Requires a bachelor’s degree in a job related field and 3 or more years of job related experience, which may be substituted by an equivalent combination of job related certification, training, education, and/or experience.
Required Certification, Licensure/Other Credentials
* Current licensure as Physician Assistant with SC LLR, in good standing.
Preferred Qualifications
* Prefer at least one year of experience working in a physician practice.
* Prefer experience coordinating care of patients across different areas/services of a health system.
Knowledge/Skills/Abilities
* Ability to communicate effectively.
* Ability to assess, identify, and address individual needs.
* Knowledge of or the ability to learn medical terminology, use medical equipment and follow clinical orders.
Job Duty
Serves as the primary point of contact for all primary care patient referrals within the UBHN, taking calls from providers/offices and connecting with patients.
