Position: Pharmacy Technician Med Reconciliation
Location: Highland Park Hospital
Full Time: 40 hours/week
Hours: Monday-Friday, 10 Hour Shifts, 4 Shifts each Week (11am - 9:30pm, 12pm - 10:30pm), Rotating Every 3rd weekend (12pm - 10:30pm)
Required Travel: No
What you will do:
Assist in the process of collecting Home Medication History of patients being admitted to the hospital. This will require using the resources provided to identify recent outpatient medication dispenses, interviewing the patient, contacting pharmacies, and accurately documenting the collected information. This work will primarily be conducted in the Emergency Department but may also include inpatient floors. Works only under the supervision of a registered pharmacist
What you will need:
Education (Required):
High school diploma or GED equivalent
Certifications (Required):
Current License as a Pharmacy Technician in the State of Illinois
One of the following must be met:
Current Pharmacy Technician Certification or must obtain CPhT (Certified Pharmacy Technician) within 90 days of hire, or
Successfully passed the Pharmacy Technician Certification Board (PTCB) or the Exam for the Certification of Pharmacy Technicians (ExCPT) examination pending certification, or
Current Pharmacy Technician license with “Student” designation issued by the State of Illinois
