POSITION SUMMARY:
Responsible for assisting in the planning, policy development, organization and direction of the Social Works Program. Provides supervision and consultation to social work staff and allied professional personnel regarding the provision of services to meet the social and emotional needs of residents and families in accordance with federal, state and local regulations and as prescribed by professional standards (in the absence of the Director).
ESSENTIAL FUNCTIONS AND RESPONSIBILITIES:
- Responsible for the supervision of four (4) Caseworkers and one (1) Administrative Assistant.
- Directs the Social Works Program in the absence of Social Work Director.
- Consults and coordinates with other facility programs regarding program planning and policy development to render the optimum of resident care.
- Assists in developing and maintaining an up-to-date policy and procedure manual regarding the provision of services to meet the social and emotional needs of residents and families in accordance with federal, state, and local regulations and as prescribed by professional standards.
- Consultation to and supervision of Social Service personnel.
- Responsible for consultation to the interdisciplinary team regarding provisions for the social and emotional needs of the resident and family.
- Responsible for the coordination of admission of residents to the facility; readmission of residents to the facility; discharge of residents to a less restrictive level of care; and resident room changes; and overall caseload responsibility for Short and Long Term Residents.
- Reviews all admission residents' PASARR’s for completeness. Reports all Target residents as mandated by regulatory agencies.
- Ensures that residents' PASARR’s are up-dated as outlined by regulatory agencies policies and procedures.
- Maintains communication with a wide variety of community health and service resources.
- Develops and conducts both formal and informal educational programs related to Residents' Rights, Advance Directives as well as confidentiality and other social and emotional needs of the residents and their families.
- Responsible for an adequate record system for obtaining, recording and filing of social work data.
- Provides counseling to residents and families regarding facility policies and procedures, feelings concerning disability, death or dying or other emotional, mental, social needs.
- Handles resident caseload and corresponding documentation as directed.
- Reports all complaint and residents rights violations to administration and involved facility disciplines. Assists with reviewing the investigation findings and developing a plan to prevent reoccurrence.
- Attends continuing education in-services and actively investigates new trends and techniques in health care practices related to the social and emotional needs of residents and families.
- Attends the monthly Quality Improvement committee. Reports on relevant audit compliance/non-compliance/corrections and changes in regulations, guidelines and practices concerning the field of Social Work.
- Performs other duties as requested by Supervisor.
QUALIFICATIONS REQUIRED:

- Bachelor’s Degree in Social Work or equivalent experience required with three to five years' experience in LTC setting.
