Advocate Trinity treats more than 90,000 patients each year, giving them access to more than 300 physicians with expertise in more than 50 specialties. This includes advanced heart attack care, primary stroke care center, oncology center and an array of surgical specialties not found in most community hospitals.
We’re Looking For:
Licensure or Certifications
- License/Registration/Certification: IL locations- LSW Eligible, LCSW preferred issued by the State of Illinois
- WI locations - Social Work Certification issued by the State of Wisconsin
Degrees:
- Master’s degree in social work from a Council on Social Work Education (CSWE) accredited school.
- Years of Experience: 1 year of social work experience in healthcare
What You’ll Do:
Schedule: Full time, 1st shift, Monday - Friday, with weekend rotation, also some holiday coverage
As an Inpatient Medical Social Worker, you will:
- Deliver comprehensive social work support by conducting psychosocial assessments, identifying patient needs, implementing appropriate interventions, follow-up care, and discharge planning.
- Collaborate with interdisciplinary teams to coordinate safe, timely care across the continuum, including post-acute services and community resources, while advocating for patient-centered care.
- Educate patients and families on healthcare decisions, available resources, and complex issues such as advanced directives, end-of-life care, and social concerns.
- Connect individuals to community services and agencies, ensuring continuity of care and smooth transitions.
- Support care coordination by managing length of stay, reducing readmissions, and ensuring appropriate levels of care within network systems.
- Maintain clear communication, thorough documentation, and compliance with regulatory and professional standards, while also contributing to staff education and professional development.
- Demonstrate knowledge of social work principles and methods and the ability to apply these in a health care setting.

Physical Requirements and Working Conditions
- This position requires travel, therefore, will be exposed to weather and road conditions.
- Operates all equipment necessary to perform the job.
- Exposed to a normal office environment.
- Exposed to patient care environment. May be exposed to hazardous materials and life-threatening diseases, therefore team members must abide by personal protective equipment as ordered.
- Weekend, holiday, and evening coverage per site requirement.
- Position requires sitting, standing, speaking, reading/writing, and walking throughout the workday.
- Fast paced work environment with established time constraints per site.
