Position Summary
We are seeking a compassionate, organized, and experienced Director of Social Services to lead and oversee social service operations within our long-term care facility. The ideal candidate will advocate for resident well-being, support families through care transitions, and ensure compliance with all social service regulations and standards.
This position plays a vital role in enhancing residents' quality of life through assessment, counseling, care planning, and coordination of services.
Key Responsibilities
- Conduct psychosocial assessments and update care plans based on resident needs
- Provide counseling and supportive services to residents and families
- Assist with admission, discharge, and transfer processes, ensuring appropriate communication with physicians and families
- Facilitate discharge planning, including coordination with community resources and services
- Advocate for resident rights and provide education to staff on psychosocial issues
- Document social services interventions and progress in resident records
- Participate in interdisciplinary care conferences and survey preparation
- Supervise and support social services staff as applicable
- Ensure compliance with federal, state, and facility regulations related to social services
Skills & Competencies
- Knowledge of long-term care social work practices and regulations
- Strong communication and counseling skills
- Ability to work effectively with residents, families, and interdisciplinary staff
- Excellent organizational and problem-solving abilities
- Ability to maintain confidentiality and professionalism in sensitive situations
Qualifications
Required
- Bachelor's degree in Social Work required
- Licensed Social Worker in the state of Michigan
Preferred
- Experience in long-term care or healthcare social services
- Leadership or supervisory experience in a healthcare setting
Physical & Work Requirements
- Ability to move freely throughout the facility and interact with residents and families
- Occasional lifting of up to 40 lbs and pushing wheelchairs as needed
