2nd Shift: 16 hours **Registered Nurse- Rate of Pay: $46.37 per hour Weekend Differential Pay $47.02
Scope of Service: The Nurse's primary function is to provide assessments, consultation, education and coordination of medical care to all clients of Continuum's residential programs. The Nurse's role is also to employ strategies to promote education and in service training to staff on any and all medical issues pertaining to client needs. We also ask that the Nurse be available strictly for consultation and in-service training to other programs as needed within the Residential Services Divisions.
ESSENTIAL JOB FUNCTIONS
- Plans and provides psychiatric nursing care. Incorporates nursing goals and interventions with behavioral health team treatment plan.
- Exercises appropriate nursing principles, procedures, and techniques used in the care and treatment of the persons served.
- Follows psychiatrist recommendations for treatment interventions.
- Teaches program staff regarding client comorbidities and provides resources and education, as necessary.
- Observes client selfadministration of medication while training new staff members on medication observation and education procedures.
- Has strong knowledge of psychotropic and medical medications and effects on persons served.
- Documents reactions to psychotropic drugs and other medications as directed by MD/APRNs.
- Conducts intake assessments with clients and ongoing nursing notes consistent with agency procedures.
- Knowledgeable of and carries out agency medication practices.
- Conducts medication reconciliation with MD/APRN; oversees the accuracy of
the Medication Observation Record; transcribes medication records in conjunction with staff; and conducts investigations for inaccuracies.
- Involves natural and holistic supports in the treatment process, as appropriate.
- Provides education for clients and staff regarding medical and psychiatric care.
- Provides education regarding preventative care and healthcare alternatives.
- Consults as part of the interdisciplinary team.
- Coordinates with pharmacies and insurance companies for proper reimbursement.
- Coordinates with case manager to liaise community referrals, as needed.
- Provides crisis intervention and support as clinically indicated.
- Completes other designated environmental tasks as assigned.
Environmental Responsibilities:
- Assists in maintaining a safe and secure environment for staff and clients.
- Maintains a clean, wellorganized work environment.
- Ensures that all egresses and walkways are clear to support workplace safety.
- Notifies supervisor of maintenance and/or safety issues and generates work orders as needed.
- Conducts and participates in fire and emergency evacuation drills, and reviews the emergency preparedness plan as outlined in agency policy.
- Completes other designated environmental tasks as assigned.
Quality Assurance:
- Adheres to policies and practices as outlined in staff handbook and program operations manual.
- Works in a respectful, constructive, and cooperative manner with fellow staff and providers.
- Upholds all Health Insurance Portability and Accountability Act (HIPAA)
regulations and secures all necessary releases of information for outside communication.
- Models culturally competent behavior interacting with clients, professionals/providers, and fellow staff.
- Maintains accurate records and case files as assigned and in accordance with programming, contractual, licensing, and accreditation requirements.
- Adheres to the standards of the Council on the Accreditation of Rehabilitation
Facilities.
- Participates in agency committee work when available or as permitted.
- Completes all trainings as required per program contract and agency policy.
- Attends agency sponsored inservices, outside trainings, conferences and
meetings to facilitate continued professional growth and ensure the quality of case management practices.
Qualifications:
- Registered Nurse with professional CT license.
- Prior experience working with special populations, including mental health,
developmental/intellectual disabilities, homelessness, substance use, and veterans preferred.
- Current certification in First Aid and CPR or willingness to obtain certification within 3 months of hiring.
- Valid Connecticut Driver’s license with clean driving record is required.
- Knowledge of the community support system and resources is preferred.
- A healthy role model with the ability to communicate healthy lifestyle practices and behavior modifications to clients is required.
- Basic computer skills required.
- Knowledge of the basic principles of psychiatric rehabilitation is preferred.
