The ideal candidate will be committed to delivering comprehensive primary and preventive care to adult patients, managing chronic conditions, and promoting overall wellness in a culturally diverse community. Creole-speaking candidates are strongly preferred.
Responsibilities:
- Provide primary and preventive care for adult patients, including physical exams, screenings, and chronic disease management
- Diagnose and treat acute and chronic medical conditions
- Collaborate with multidisciplinary teams to coordinate patient-centered care
- Counsel patients on lifestyle modification, disease prevention, and medication management
- Maintain accurate, timely documentation in the electronic health record (EHR)
- Participate in quality improvement initiatives and team meetings
- Complete additional tasks as directed by supervisor
Qualifications
- Medical degree (MD, DO, NP, or PA) from an accredited institution
- Board certification or eligibility in Internal Medicine or Adult Primary Care
