TO PROVIDE CASE MANAGEMENT SERVICES, INCLUDING: PATIENT ASSESSMENT, FAMILY EDUCATION AND INTERVENTION, COLLATERAL CONTACT WITH MENTAL HEALTH PROVIDERS, AND DISCHARGE PLANNING FOR THE PSYCHIATRIC PATIENTS ON THE INPATIENT SERVICE, CONSISTENT WITH THE HOSPITAL'S MISSION STATEMENT, POLICIES AND PROCEDURES. SCOPE OF PATIENT CARE SERVICES PROVIDED ENCOMPASS AGE GROUPS RANGING FROM ADOLESCENTS TO GERIATRICS. Provide specialized group programming for all 3 units.
EOW rotation 32hrs per week.
Location: Valley Health Systems · Geriatric Behavioral Health - S1
Schedule: Full Time, day shift, 8am-4pm
