Legacy Healing Center – Los Angeles & North Hollywood, CA
In-Office | Full-Time | $75,000 Salary
Legacy Healing Center is a premier substance abuse treatment facility committed to providing compassionate, high-quality care to individuals on their recovery journey. We are currently seeking a dedicated and organized Case Manager to support our Los Angeles and North Hollywood locations.
This is an in-office position requiring flexibility to travel between nearby facilities and work one of the following schedules:
Tuesday- Saturday
Position Overview
The Case Manager plays a vital role in coordinating care, supporting client needs, and ensuring continuity throughout treatment. This position works closely with clinical teams, external providers, and clients to facilitate successful treatment outcomes and smooth transitions of care.
Key Responsibilities
- Coordinate and manage client care plans from admission through discharge
- Assist clients with resources related to housing, employment, legal matters, and aftercare planning
- Maintain accurate and timely documentation in compliance with regulatory and company standards
- Communicate effectively with clinical staff, admissions, utilization review, and external partners
- Facilitate referrals and follow-up appointments as needed
- Travel between Legacy Healing Center locations in the Los Angeles area as required
- Advocate for clients while maintaining professional boundaries and ethical standards
Qualifications
- Bachelor’s degree required
- RADT certification required (California)
- Prior case management experience in substance abuse or behavioral health preferred
- Reliable transportation and ability to travel between local offices
- Strong organizational, communication, and time-management skills
- Ability to work independently in a fast-paced, compliance-driven environment
Schedule & Compensation
- Full-time, in-office position
- Schedule options: Tuesday–Saturday
- Salary: $75,000 annually
