We are seeking a highly skilled and experienced Clinical Supervisor to join our team at Uplift Home Care. As a Clinical Supervisor, you will be responsible for overseeing and coordinating the clinical operations within our organization. You will provide guidance and support to our clinical staff, ensuring the delivery of high-quality patient care.
Responsibilities:
- Provide supervision and guidance to clinical staff, including nurses, medical assistants, and other healthcare professionals
- Conduct regular performance evaluations and provide feedback to staff members
- Develop and implement policies and procedures to ensure compliance with regulatory standards
- Collaborate with other departments to coordinate patient care and ensure smooth operations
- Assist in the development and implementation of quality improvement initiatives
- Conduct training sessions for staff members on new procedures or protocols
- Monitor patient outcomes and make recommendations for improvement
- Oversee discharge home planning process and ensure appropriate follow-up care for patients
- Provide direct patient care as needed
- Utilize electronic medical record systems (WellSky) to document patient information accurately
- Rotating on-call with other supervisors
Qualifications:
- Degree in nursing or related field required; RN preferred
- Active state nursing license required, compact nursing license preferred
- Minimum of 2 years of clinical experience in a healthcare setting
- Previous experience in a supervisory or leadership role preferred
- Excellent communication and interpersonal skills
- Ability to work well under pressure and make sound decisions in a fast-paced environment
