- Makes referral to appropriate resources. Coordinates care between multiple/outside Care Teams.
- Demonstrates effective client assessment and evaluation skills.
- Functions as a positive role model in professional relationships and interactions.
- Comments from MLHS employees, clients and families about positive attitude, professionalism etc.
- Maintains 5 visits and documentation in an 8 hour day unless prior approved by manager/supervisor.
- Practices infection control measures per standard.
- Reports concerns of client safety to appropriate entity and provides necessary measures to promote safety.
- Identifies needs and concern regarding safety and infection control.
- Ensures cost effective client services.
- Monitor and evaluate appropriate use of supplies.
- Adheres to MLHS policies and practices including:

+ Exhibits a positive attitude.
+ Participates in activities directed toward evaluating and improving service area functions.

Required Education And Experience
- Graduate of an accredited school of nursing with current licensure as a registered nurse with the State of Minnesota.
- Knowledge of Minnesota Nurse practice act and code of ethics.
- Previous experience desirable.
- Must successfully complete criminal background check as required by the State of Minnesota and Drug/alcohol screening per MLHS policy.
- Must have a valid driver's license.
- Must have CPR/BLS certification.
