Position Description – What The Role Entails
The essential function of the Case Manager is to provide comprehensive assessment supported housing, vocational and educational services, intensive case management, skills training, counseling, crisis intervention, and community resource development and service coordination to assist 17 1/2-20-year-old youth in residential care to achieve successful independent community living.

Areas Of Accountability - What You Will Be Doing
- Assists with the coordination of the resident’s DCFS Service Plan from admission to discharge
- Assists youth in developing educational goals and identifying an appropriate educational placement
- Assists youth in completing other goals that have been outlined in the resident’s DCFS Service Plan
- Works with support services and acts as an advocate for the resident
- Ensures that the overall delivery of services provided to the residents is effective and meets Department requirements and standards
- Provides a continuum of care for the youth in their residential placement, as well as transport/escort them to various community-based services
- Works flexible hours including some evenings and weekends based on the needs of the youth served and participates in an on-call rotation
- Attends all court hearings and administrative case record reviews
- Maintains documentation according to DCFS standards and format
- Promotes RFS’s mission to participants, staff, and the other shareholders partnerships and alliances (government, community, congregations, individuals, foundations, and corporations)
- Coordinates and monitors program policy and procedures and successfully advocates support systems on behalf of the program and its participants
- Oversees day-to-day program activities
- Ensures timely submission of all required program documentation
- Assists with the development and submission of all program funding proposals
- Provides direct supervision of Lead RA and RA Staff
- Provides regular individual supervision with supervisees
- Assists with identifying, developing, and implementing training and staff development activities
- Explores new directions for program in service delivery methods, overall programming, and fundraising opportunities
- Attends all required Agency, Department, and other staff trainings and general meetings
- All other duties as assigned

- Job requires driving more than 10 miles and be available in case of emergencies. The ability to lift 10lbs to carry case files from the file room to the desk
Schedule
Regular work hours are 9:00 am-5:00 pm, however there may be a need for flexibility contingent upon circumstances. There may be a need to work after hours and/or weekends depending on circumstance.
