Can an F-1 student on CPT work as an Occupational Therapist without a full OT license?
CPT authorization allows you to work in a clinical capacity before you hold a full state OT license, but you're typically working under supervision as a student or fieldwork trainee, not as a licensed OT. Your employer must structure the role as a supervised clinical placement. State licensing boards set their own rules on what unlicensed OT students can perform, so confirm the scope of practice with your fieldwork supervisor and your state board before you start.
Does CPT authorization cover both Level I and Level II OT fieldwork?
Yes, both fieldwork levels can qualify for CPT if your program designates them as an integral part of the curriculum and your DSO authorizes each placement separately. Level II fieldwork, which is longer and more intensive, is especially well-suited because it's typically required for degree completion. Each new placement needs its own CPT authorization on your I-20, even if it's with the same employer.
How do I find Occupational Therapist employers who are familiar with CPT authorization?
Migrate Mate lets you search OT roles and filter by employers who have a history of hiring F-1 students. That filing history signals familiarity with CPT and work authorization paperwork, which matters because healthcare employers new to CPT often create delays by not understanding the site agreement or I-20 amendment process. Starting with experienced employers saves weeks of back-and-forth.
What happens to my CPT authorization if my OT fieldwork placement ends early?
CPT is tied to a specific employer, location, and date range on your I-20. If your placement ends before the authorized end date, you must stop working immediately and notify your DSO. Working beyond the original placement without a new CPT authorization on an amended I-20 is an F-1 status violation. If you're moving to a different facility mid-program, your DSO must issue a new authorization before you start at the new site.
Does using 12 months of full-time CPT affect my OPT eligibility after graduation?
Yes. Under USCIS rules, 12 months or more of full-time CPT exhausts your OPT eligibility entirely. Part-time CPT, defined as 20 hours per week or fewer, does not count against OPT. Most OT Level II fieldwork runs full-time, so if you complete two full-time rotations of six months each, you won't qualify for post-completion OPT. Plan your CPT usage carefully with your DSO, especially if you intend to pursue STEM OPT or H-1B visa sponsorship after graduation.
