Your Impact
- Partner with care team members to develop and implement patient-centered clinical plans.
- Deliver wrap-around services through home visits and collaborative care alongside providers and RNs.
- Perform key clinical tasks in the field— chronic condition support, wound care, phlebotomy, medication review, and more.
- Engage hard-to-reach patients, bringing them vital care and support directly at home.
- Ensure continuity of care by providing coverage for team members when needed.
- Help drive better patient outcomes, lower costs, and greater patient and staff satisfaction through our interdisciplinary care model.
Requirements
- Licensed Practical Nurse (LPN) licensure in the state of employment
- Ability to conduct independent home visits
- Experience providing care for the whole person, including caring for adults and seniors with high medical and social complexity
- Current CPR and BLS certifications required
- Intermediate to advanced computer and software skills, including Patient Health Exchange Platforms, Electronic Health Records, and Outlook
- Ability and means to travel as needed in a timely manner to locations that may have limited access to public transportation; proof of liability and property damage insurance on vehicle used is required.
- DRIVER REQUIREMENTS: Licensed for a minimum of 5 years. No vehicle-related suspensions, DUI, reckless driving, leaving the scene of an accident, vehicular manslaughter, felony convictions, or 3 or more moving violations within the last 5 years. No more than 2 moving violations or 1 at-fault accident in the last 3 years.

Base Salary/ Wage Range $35.00 to $38.00/hour with annual bonus. Compensation for the role is commensurate with the candidate’s qualifications, skills, competencies, and experience and may fall outside of the range shown.
