Translate your fellowship credentials for PERM
PERM job requirements must match your actual qualifications exactly. Document your pediatric surgery fellowship, board certification status, and any subspecialty training in a credentials package your employer can cite when defining the minimum job requirements.
Target children's hospitals with GME infrastructure
Academic children's hospitals and teaching institutions already maintain immigration counsel and PERM workflows for physician recruits. Prioritize these employers during your job search since standalone private practices rarely have the administrative capacity to sponsor green cards.
Confirm the EB category before accepting an offer
Pediatric surgeons typically qualify for EB-2 as advanced-degree professionals, but some employers file under EB-3. Your category affects priority date waiting times, especially if you're from India or China, so clarify this before signing your employment contract.
Use Migrate Mate to filter sponsoring employers
Searching broadly for pediatric surgeon openings wastes time on employers who won't sponsor. Use Migrate Mate to surface roles at institutions with active green card sponsorship history, so you apply where the pathway already exists.
Verify prevailing wage tier using OFLC Wage Search
PERM requires your offered salary to meet or exceed the DOL prevailing wage for your SOC code and practice location. Check OFLC Wage Search before negotiations so you know whether your offer satisfies Level I through Level IV wage requirements for your geographic area.
Coordinate PERM timing with your J-1 or H-1B status
If you're completing a J-1 waiver or holding H-1B status, your employer can file PERM concurrently with your nonimmigrant status but cannot file the I-485 until your priority date is current. Plan the sequence with your employer's immigration attorney to avoid gaps in work authorization.

Find Pediatric Surgeon JobsPediatric Surgeon Green Card Sponsorship: Frequently Asked Questions
Does a pediatric surgeon role qualify for EB-2 or EB-3 green card sponsorship?
Pediatric surgery qualifies for both categories. EB-2 applies to advanced-degree professionals, and most pediatric surgeons hold an MD plus fellowship training that satisfies this standard. EB-3 covers skilled workers and professionals and is sometimes used when employer preference or priority date strategy makes it advantageous. Your employer's immigration counsel determines which category to file under based on the job requirements and your credentials.
How does green card sponsorship differ from H-1B sponsorship for a pediatric surgeon?
The green card path through PERM and I-140 leads to permanent residency rather than a temporary status. There's no annual lottery at the EB-2 or EB-3 level, though per-country backlogs can extend timelines for applicants from India and China. H-1B is capped at 85,000 per year and subject to lottery selection, while PERM is a regulatory process with no cap on petitions filed.
What does the PERM labor certification process look like for a pediatric surgeon position?
Your employer files an application with DOL certifying that no qualified U.S. worker is available for the role at the prevailing wage. This requires a defined recruitment period with documented advertising, typically lasting 30 to 60 days. Once DOL certifies the PERM, your employer files the I-140 immigrant petition with USCIS. The entire PERM-to-I-140 stage commonly takes 12 to 24 months before adjustment of status can begin.
Where can I find pediatric surgeon jobs where employers are already set up to sponsor green cards?
Migrate Mate lets you search pediatric surgeon openings filtered by employers with green card sponsorship history, so you're not cold-applying to institutions that have never navigated PERM. Children's hospitals, academic medical centers, and large multispecialty health systems are the most common sponsors for this role, and Migrate Mate surfaces those opportunities directly.
Can a pediatric surgeon start the green card process while on a J-1 waiver or H-1B?
Yes. PERM can be filed while you hold J-1 waiver or H-1B status. Filing PERM and I-140 early locks in your priority date, which matters if your country of birth has a backlog. You can't file I-485 for adjustment of status until your priority date is current per the USCIS Visa Bulletin, but having an approved I-140 preserves your place in line and may allow H-1B extensions beyond the standard six-year cap under AC21 portability rules.
