Verify your credentials meet ECFMG requirements
PERM filings for Critical Care Physician roles require U.S. board certification or active candidacy. Confirm your ECFMG certification and USMLE completion are documented before approaching employers, since incomplete credentials stall PERM at the recruitment-audit stage.
Target hospitals with active PERM filing history
Academic medical centers, large health systems, and VA facilities file PERM far more frequently than private practices. Search DOL's OFLC disclosure data by SOC code for Critical Care Physicians to identify employers who have successfully sponsored this role before.
Search sponsoring employers on Migrate Mate
Use Migrate Mate to filter Critical Care Physician openings by employers with documented EB-2 and EB-3 sponsorship history, so you're applying to roles where green card support is already established rather than negotiating it from scratch.
Clarify prevailing wage tier during offer negotiation
DOL assigns your role a Level I through IV wage using the OFLC Wage Search. Employers sometimes file at Level I to reduce costs, which can trigger audits. Confirm the wage level before signing an offer, since it affects both your salary floor and PERM defensibility.
Understand the EB-2 versus EB-3 priority date impact
If you hold a U.S. fellowship or subspecialty board certification, your employer can file EB-2, which often carries shorter wait times for many nationalities. EB-3 is available without an advanced-degree distinction but may have longer backlogs for applicants from high-demand countries.
Plan for concurrent I-140 and I-485 filing windows
Once your employer's PERM is certified, request that they file the I-140 with USCIS promptly. If a visa number is immediately available, concurrent I-140 and I-485 filing lets you apply for adjustment of status before the backlog moves, locking in your priority date.
Green Card Critical Care Physician: Frequently Asked Questions
Does a Critical Care Physician role qualify for EB-2 or EB-3 green card sponsorship?
Critical Care Physician positions qualify for both EB-2 and EB-3 sponsorship depending on how the employer structures the role. EB-2 applies when the position requires an advanced degree beyond an MD, such as a fellowship or subspecialty certification. EB-3 covers roles where a professional degree alone satisfies the job requirements. Your employer chooses the category when filing the PERM labor certification with DOL.
How does PERM green card sponsorship differ from H-1B for Critical Care Physicians?
H-1B visa is a temporary work visa subject to an annual lottery cap and requires renewal every three years. PERM-based green card sponsorship has no annual cap at the petition level and leads to permanent residency rather than temporary status. The process takes longer overall, typically two to four years from PERM filing to a green card for most nationalities, but it eliminates the uncertainty of lottery selection and provides a permanent path to stay and practice in the U.S.
Which types of employers sponsor green cards for Critical Care Physicians most often?
Large academic medical centers, university-affiliated teaching hospitals, and multi-site health systems sponsor Critical Care Physician green cards most frequently because they have in-house legal infrastructure to manage PERM filings. VA medical centers and some large regional hospital networks also have established sponsorship programs. Smaller private practices and independent groups rarely sponsor because of the administrative burden and cost involved in the PERM process.
Where can I find Critical Care Physician jobs with confirmed green card sponsorship?
Migrate Mate lets you search Critical Care Physician openings filtered by employers with documented EB-2 and EB-3 green card sponsorship history, so you can focus your applications on roles where sponsorship is already part of the hiring process rather than negotiating it case by case after an offer.
What happens to my green card case if I change employers before receiving my green card?
If you leave an employer before your I-485 adjustment of status is approved, your PERM and I-140 are generally tied to that employer and do not transfer automatically. If your I-140 has been approved for more than 180 days and your I-485 has been pending for at least 180 days, AC21 portability may allow you to transfer your priority date to a new employer in a same or similar Critical Care Physician role. Consult an immigration attorney before making any job change mid-process.
