ROLE AND RESPONSIBILITIES
This position maintains medical record integrity by supporting filing, prepping, and scanning, indexing and quality assuring paper documentation into the patient’s electronic medical records.
- Reconcile all patient information with the documents received for both the hospital and physician practice documentation.
- Remove any documents that do not belong to the identified patients.
- Prepare discharge paper records according to the organizations standards for scanning.
- Scan all prepped legal medical record documentation into OnBase following in the established procedures.
- Indexes scanned pages into appropriate document type and patient’s chart.
- Quality controls all scanned documents prior to releasing to the patient’s legal medical record.
- Perform all duties at expected production and quality standards.
- Organize and file organizations records including but not limited to inpatient, outpatient, ED, loose ancillary reports and microfiche medical records.
- Ensures that all medical records contain information on a simple patient only.
- Provides document correction support as needed.
- Assists with reconciling interface related issues to ensure all documentation resides in the patient’s record.
- Organizes and empties yellow transport bags and distributes documents received.
- Demonstrates support for the mission, values and goals of the organization through behaviors that are consistent with Connecticut Children’s standards.
- Creates, organizes and maintains legacy patient medical record charts for all patients seen in the practice area.
- Prepare patient charts to ensure availability for providers prior to medical services.
- Utilizes patient look up to locate patient information missing from chart.
- Ensure JCAHO and HIPAA compliance.
- All other support duties as assigned.
MINIMUM QUALIFICATIONS
- Minimum education: High School diploma or equivalent required.
- Minimum experience: One year clerical experience.
- Preferred education & experience: Three years’ experience in a Health Information Management Department preferred.
LICENSE AND/OR CERTIFICATION
- Required: None.
- Preferred: HIM Fundamental Certification.
KNOWLEDGE, SKILLS, AND ABILITIES
Knowledge of:
- Scanning operations and equipment/hardware.
- Epic software.
- OnBase Software.
- Terminal digit filing recommended.
- HIPAA guidelines including Protected Health Information privacy requirements.
Skilled at:
- Good interpersonal, numerical and organizational skills.
- Computer skills.
- Demonstrates proficiency in Microsoft Word and Excel.
- Time management skills.
Ability to:
- Read, write and speak English proficiently.
- Maintain confidentiality.
- Ability to build and maintain working relationships with team members.
- Effectively multi-task.
- Readily support new processes.
- Adapt to a fast-paced environment.
- Adjust to schedule changes and covering on-site locations.
