Responsibilities

• Conduct thorough patient assessments including vital signs, physiological evaluations, and medical history reviews to determine care needs.
• Deliver personalized nursing care in patients’ homes, including wound management, medication administration, and disease management.
• Collaborate effectively with physicians, case managers, social workers, and other healthcare providers to develop and implement individualized care plans.
• Manage patient records accurately using Electronic Health Records (EHR) systems while ensuring compliance with HIPAA regulations.
• Educate patients and families on health conditions, medication use, discharge planning, and post-acute care strategies to promote self-care.
• Monitor patient progress through ongoing evaluation and adjust care plans accordingly to optimize health outcomes.
• Coordinate services such as hospice care, post-acute recovery, or chronic disease management while managing caseloads efficiently.

Qualifications

• Valid nursing license with current registration in the applicable state.
• Proven experience in critical care environments such as ICU, emergency rooms, or trauma centers; Level I or Level II trauma center experience preferred.
• Strong background in hospital medicine, medical-surgical units, or home health nursing with acute care experience.
• Knowledge of infectious disease protocols, geriatrics, behavioral health, and post-acute care practices.
• Proficiency in EMR/EHR systems and understanding of healthcare regulations including HIPAA and health policy standards.
• Excellent patient service skills with the ability to perform comprehensive patient assessments and clinical case management.
• Ability to communicate effectively with diverse patient populations and collaborate seamlessly with multidisciplinary teams.
• Familiarity with diagnostic evaluation techniques, managing patients as a nurse case manager, and discharge planning processes.
