--SHIFT CARE NURSE (LPN/RN)--

Position Overview:
As a Home Health Care Nurse at ATM Home Health Care Services, you will be at the forefront of our patient care services, providing essential medical and personal support to individuals in their homes. Your role will be crucial in helping patients manage their health and improve their quality of life.
Key Responsabilities:
- Patient Assessment: Conduct thorough assessments to understand each patient's needs and create tailored care plans.
- Medical Administration: Administer medications and treatments with precision and care.
- Health Monitoring: Keep a close eye on patient conditions, noting any changes and reporting them to the healthcare team promptly.
- Patient Education: Empower patients and their families with knowledge about their conditions, medications, and lifestyle adjustments.
- Daily Living Assistance: Aid patients with everyday activities, ensuring their comfort and well-being.
- Record Keeping: Maintain detailed and accurate patient records for seamless care continuity.

Qualifications:
- LPN/RN License (Required)
- Accepting new graduates, with a preference for home health care.
- Strong clinical and problem-solving skills.
- Excellent communication abilities.

- Ability to travel to patient homes within the region.
- Familiarity with Electronic Medical Records (EMR) systems.
Credentialing Requirements for both positions:
- 2 forms of identification (State ID, Driver’s License, Social Security and/or Passport)
- Physical Exam (document within 1 year is accepted)
- PPD/ QuantiFERON TB Gold / Chest X-Ray
- Hepatitis Vaccine (or sign our declination form)
- Flu Vaccine (or sign our declination form)
- Sign a Non-Driver Waiver or Provide Auto Registration and Insurance
- Criminal Background Check (document within 1 year is accepted)
- FBI Fingerprint (document within 2 years is accepted)
- Child Abuse Clearance (document within 1 year is accepted)
- Valid LPN/RN License in the state of Pennsylvania
- Liability Insurance (RN/LPN)
- Drug Test (available in the office)

Position Overview:
We are seeking a compassionate and detail-oriented Clinical Nurse (In Office) to join our healthcare team. This role combines both in-office responsibilities and fieldwork, providing essential clinical support to patients in their service locations. The Clinical Nurse is responsible for conducting patient visits for various care milestones, supporting ongoing clinical needs, and ensuring the highest standard of care is delivered in compliance with clinical guidelines.
Key Responsibilities:
* Conduct in-person patient visits for:
- Admissions: New patient onboarding and initial evaluations
- Recertifications: Comprehensive assessments every 60 days
- Resumptions of Care: Following hospital discharge or care interruption
- Supervision Visits: Ongoing assessments every 14 days to ensure quality of care and compliance
- Other visits: Directed by the administrator or Clinical Manager
* Respond to daily clinical inquiries from field staff, physicians, and patients
* Document all clinical findings and patient interactions accurately and in a timely manner
* Collaborate with interdisciplinary teams to update care plans and coordinate services
* Ensure compliance with federal, state, and organizational standards for patient care
* Attend internal meetings and training sessions as required
Qualifications:
- Current Registered Nurse (RN) license in the state of Pennsylvania
- Minimum 1 year of clinical nursing experience, preferably in home health or community-based care
- Strong clinical assessment, critical thinking, and communication skills
- Ability to work independently and manage a flexible field-based schedule
- Must have a reliable vehicle, valid driver’s license, and willingness to drive to patient service locations
Work Environment:
- Office-based administrative and clinical tasks
- Field visits to patient homes or service locations (travel required)
- Use of electronic medical records (EMR) system for documenting assessments
