J-1 Visa NP Jobs
NP roles in the United States are available to international healthcare professionals through the J-1 visa under the Exchange Visitor Program, typically via the Research Scholar, Specialist, or Clinical Trainee categories depending on your qualifications and program structure. Finding a host employer that works with a State Department-designated sponsor is the first step toward securing sponsorship and a DS-2019.

Get Access To All JobsTips for Finding J-1 Visa Sponsorship in NP
Verify your NP credentials meet U.S. licensure standards
Most host employers require NCLEX-RN passage and state NP licensure before they'll engage a designated sponsor. Confirm your nursing board's credential recognition policy early, as foreign transcript evaluations through CGFNS can take several weeks.
Distinguish Trainee from Specialist category eligibility
NPs within five years of their degree typically qualify under the Trainee category, capped at 18 months. Those with longer post-graduate experience pursuing advanced clinical education may fit the Specialist category instead. Ask prospective host employers which category their sponsoring organization supports.
Target health systems with existing Exchange Visitor programs
Academic medical centers and federally qualified health centers are far more likely to have established relationships with State Department-designated sponsors. Use Migrate Mate to filter NP roles at institutions that have structured J-1 host agreements already in place.
Get the training plan signed before your DS-2019 is issued
The designated sponsor, not your host employer, issues the DS-2019, but the training plan must be co-signed by both parties first. Delays almost always stem from the host failing to return the signed plan on time, so build two weeks of buffer into your start-date negotiations.
Clarify two-year home residency requirement exposure upfront
J-1 NPs funded by their home government, or working in a U.S. government-designated shortage area, may be subject to the two-year home residency requirement. Determine this before accepting an offer, since it affects any subsequent H-1B or green card pathway from the same role.
Confirm the host employer's USCIS compliance obligations
J-1 host employers must maintain accurate records, respond to monitoring visits from the designated sponsor, and report any material changes in your training program. Ask HR to confirm these responsibilities are documented, as gaps in compliance can trigger DS-2019 revocation mid-program.

Find NP JobsNP J-1 Visa: Frequently Asked Questions
Which J-1 program category typically applies to NP roles?
It depends on your experience level and program structure. NPs pursuing structured post-graduate clinical training within five years of their degree usually fall under the Trainee category, which allows up to 18 months. Those with more advanced credentials conducting specialized clinical or research work may qualify under the Specialist or Research Scholar category. Your designated sponsor determines the final category based on your documented training objectives.
Who actually sponsors the J-1 visa for an NP, the hospital or a separate organization?
The hiring hospital or clinic is the host employer, not the visa sponsor. The J-1 visa sponsor is a U.S. Department of State-designated organization, such as ECFMG, Cultural Vistas, or another approved body, that issues the DS-2019 form and oversees program compliance. The host employer must have a formal agreement with one of these designated sponsors before your exchange visitor status can be activated.
Can an NP on a J-1 visa transition to an H-1B or green card afterward?
Potentially, but the two-year home residency requirement can block that path if it applies to you. NPs whose J-1 program was funded by their home government or who worked in a designated shortage area are typically subject to this requirement and must return home for two years, or obtain a waiver, before changing to H-1B or adjusting status. Confirm your waiver exposure before accepting any J-1 role.
How do I find NP host employers that already work with J-1 designated sponsors?
Use Migrate Mate to search NP roles filtered by J-1 sponsorship alignment. Academic medical centers, teaching hospitals, and federally qualified health centers are the most common host employer types because they maintain existing relationships with designated sponsors. Targeting these institutions directly reduces the risk of an employer who is unfamiliar with the DS-2019 process declining to participate mid-hire.
Does the J-1 Trainee category allow NPs to provide direct patient care?
The Trainee program is structured around education and training, not independent clinical practice. NPs under the Trainee category can participate in supervised clinical activities as part of a formal training plan, but the program must be designed primarily around skill development, not service delivery. Host employers and designated sponsors review training plans carefully to ensure they meet this distinction, since programs oriented toward filling staffing needs can be rejected.
