Get Access To All JobsTips for Finding J-1 Visa Sponsorship as a Physician
Verify your ECFMG certification before applying
ECFMG certification is a prerequisite for J-1 Physician sponsorship, not a parallel step. Confirm your USMLE scores, medical school credentials, and identity documents are on file with ECFMG before contacting any residency or fellowship program.
Target ACGME-accredited programs with IMGs on staff
J-1 Physician sponsorship is only available at training programs accredited by ACGME. Programs that have previously matched international medical graduates are far more familiar with the DS-2019 process and host obligations than programs that haven't.
Clarify the two-year home residency rule upfront
Most J-1 Physicians are subject to the two-year home country physical presence requirement, which bars H-1B or green card status until fulfilled or waived. Ask programs how they support Conrad 30 or federal agency waiver applications before accepting an offer.
Use Migrate Mate to find physician roles at J-1-ready hosts
Not every hospital advertising an open physician training position has experience hosting J-1 exchange visitors. Search Migrate Mate to identify U.S. health systems and academic medical centers actively seeking physicians under J-1 sponsorship arrangements.
Negotiate your training plan before the DS-2019 is issued
ECFMG requires a detailed training plan signed by both you and your program director before issuing the DS-2019. Resolve scope, duration, and rotation structure with your host before submitting the sponsorship application to avoid delays or amendments.
Understand the 7-year time limit across J-1 categories
The J-1 Physician program caps aggregate training time, including any prior J-1 participation, at seven years. If you've held a J-1 in another category previously, calculate your remaining eligibility before committing to a multi-year fellowship track.

Find Physician JobsPhysician J-1 Visa: Frequently Asked Questions
Which J-1 program category applies to international medical graduates doing residency or fellowship training?
International medical graduates pursuing residency or fellowship training in the United States fall under the J-1 Physician program category. ECFMG is the sole U.S. Department of State-designated sponsor for this category. It is separate from the Research Scholar or Trainee categories, which apply to other professional contexts. Your host program must be ACGME-accredited for this category to apply.
Who actually sponsors the J-1 visa for a physician, the hospital or ECFMG?
ECFMG is the designated sponsor, meaning it issues your DS-2019 form and is legally responsible for your program compliance under State Department regulations. The hospital, academic medical center, or health system where you train is the host organization. Confusing the two is common, but the host cannot independently sponsor your J-1 status. ECFMG must approve the arrangement first.
Does the J-1 Physician visa carry a two-year home residency requirement?
Yes, the vast majority of J-1 Physician exchange visitors are subject to the two-year home country physical presence requirement under INA Section 212(e). This means you must return to your home country for two years after completing your program, or obtain a waiver, before you can change to H-1B status or apply for a green card. Conrad 30 and federal agency waivers are the two main pathways.
How do I find U.S. hospitals and health systems that are set up to host J-1 physicians?
Not every program advertising open positions has prior experience with J-1 hosting requirements or the ECFMG sponsorship process. Migrate Mate lets you filter for physician roles at U.S. employers who have experience with J-1 exchange visitors, saving you from pursuing programs that will stall at the sponsorship stage.
Can I work clinically outside my J-1 training program at a second site?
No. J-1 Physician exchange visitors are authorized to train only at the host site listed on the DS-2019 and approved by ECFMG. Any clinical activity at a second location, even unpaid or informal, is outside the scope of your program and violates your J-1 status. If your training requires rotations at affiliated sites, those must be documented in your training plan and approved by ECFMG before you begin.
