Summary
A Transplant Social Worker is a specialized professional that is an integral member of a multidisciplinary transplant team. The incumbent is responsible for psychosocial screening, assessment, diagnosis, treatment, and follow-up for transplant patients and living donors. In collaboration with physicians, transplant coordinators, TFC, PHS providers, and other community agencies, they assist in the management of psychosocial needs of the patients through all phases of transplantation and living donation. This position is a key member of the multidisciplinary transplant healthcare team and is directly responsible for supporting and maximizing the social functioning and adjustment of the patient and their family during the transplant experience.

Responsibilities:
A Transplant Social Worker is a specialized professional that is an integral member of a multidisciplinary transplant team. The incumbent is responsible for psychosocial screening, assessment, diagnosis, treatment, and follow-up for transplant patients and living donors. In collaboration with physicians, transplant coordinators, TFC, PHS providers, and other community agencies, they assist in the management of psychosocial needs of the patients through all phases of transplantation and living donation. This position is a key member of the multidisciplinary transplant healthcare team and is directly responsible for supporting and maximizing the social functioning and adjustment of the patient and their family during the transplant experience.
- Facilitates effective communication between administration, staff, physicians, other department, patients, families, and the community.
- Exemplifies a high standard of leadership qualities and professionalism. Mentors the staff in developing and achieving professional goals. Participates in conflict resolution as necessary and acts as an effective problem solver.
- Conducts thorough and accurate transplant patient psychosocial evaluation that will establish an individual care plan to maximize transplant recovery while reducing the impact of psychosocial risk factors and engaging.
- Proficient at Psychosocial assessment documentation in patients Electronic Medical Record and ensures regulatory compliance.
- The Transplant Social Worker is responsible for the identification and assessment of psychosocial problems and will work with patients and families to achieve resolution or will provide referrals to the appropriate agencies for services.
- The Transplant Social Worker shall be available for hospitalized patients and their families throughout the episode of illness in the hospital. This includes psychosocial problem-solving activities in all phases of transplantation and assessment of financial needs and available resources, in conjunction with the Transplant Financial Coordinator, to ensure an optimal outcome.
- The Transplant Social Worker functions as an advocate and liaison to the multidisciplinary healthcare team, actively participates in patient selection committees, multidisciplinary rounds, discharge planning, and transplant education, to achieve individualized optimal patient outcomes.
- The Transplant Social Worker collaborates as a key member of the multidisciplinary healthcare team. Collaboration includes the active participation in team decision-making processes, creative problem-solving approaches to address complex situations and utilization of critical thinking and conflict resolution skills to effectively balance competing needs and responsibilities.
- The Transplant Social Worker acts as an expert resource and contact for the multidisciplinary team, including other social workers, community case managers, physicians, ancillary staff and nursing staff on a multi-state and regional basis to help promote the optimal continuity of care for transplant patients and living donors.
- Maintains current knowledge of information in related field through seminars, workshops, and publications.
- Requires knowledge of current CMS, UNOS/OPTN regulatory requirements. Understands Medicare, Managed care and third-party payer requirements related to transplant.
- Demonstrated ability to communicate effectively orally and in writing.
- Strong interpersonal skills enabling effective team collaboration, establishment of sound professional relationships with patients and families, and tolerance of job-related psychological stress needed.
- Assures implementation of change projects/initiatives.
- Participates in all policies, records, data and reports as necessary for satisfaction of State, Federal, JCAHO CMS and OPTN regulations.
- Other duties as assigned.
Qualifications
- Masters degree in Social Work Required.
- Licensed Master Social Worker Required. - LMSW
- Current license from the State of New Mexico as a Licensed Independent Social Worker, Licensed Clinical Social Worker, or Licensed Medical Social Work license.
- 5 years hospital social work or discharge planning experience in relevant setting required.
- Experience in transplant, inpatient hospital social work or discharge planning preferred.
