VNA is looking for a Medical Social Worker.
Job Summary: The Medical Social Woker evaluates the social and emotional factors affecting a patient’s illness, care needs, ability to function, and capacity to cope with challenges in daily living with our Home Health and Palliative Care patients. Collaborates with the healthcare team to develop individualized treatment plans and actively participates in patient care conferences.
Status: Per Diem
Education/Experience/Skills/Qualifications:
- Bachelor of Science in Social Work with at least one year of healthcare experience, or a Master’s degree in Social Work
- At least on year of social work experience in a healthcare setting, preferably in home health
- Possess a current CPR certification
Special Requirements:
- Current California driver's license
- Auto insurance
- Reliable vehicle
