Advantage Home Health Services is looking for a compassionate and motivated Medical Social Worker with an active Ohio LISW license to join our home health team.
You'll work directly with patients and families, helping them navigate the social, emotional, and practical challenges that can impact their health, independence, and ability to remain safely at home.
What You'll Do
- Assess patients' social, emotional, and community-resource needs.
- Connect patients and families with appropriate community programs and resources.
- Collaborate with nurses, therapists, physicians, and other members of the care team.
- Participate in care planning and discharge planning.
- Provide support and guidance to patients and caregivers.
- Complete required clinical documentation accurately and on time.
- Communicate patient changes and concerns to the care team.
- Supervise Social Worker Assistants (SWAs) when applicable.
What We're Looking For
- Active Ohio LISW license in good standing – REQUIRED
- Master's degree in Social Work from a CSWE-accredited program.
- At least one year of social work experience in a healthcare setting.
- Strong communication, organization, and interpersonal skills.
- Ability to work independently while collaborating with a supportive clinical team.
- Ability to travel to patients' homes within the assigned service area.
